摘要
目的:探讨Ⅱ型糖尿病(diabeticmellitus,DM)患者血糖与肾损伤指标的相关性。方法:选择2010年来我院住院的Ⅱ型糖尿病患者作为研究对象,采用OLMYPUS5400全自动生化分析仪,测定其血糖(GLU)、尿糖(UGlu)、24h内生肌酐清除率(Ccr)、尿蛋白(UTP),数据用SPSS11.5forWindows进行统计分析。结果:随着血糖升高尿糖、UTP有升高趋势,血糖水平11.0-12.9mmol/L组UTP最高,为0.93mg/L。血糖水平13.0-15.0mmol/L组UGLU最高,为115.75mmol/L。血糖水平与24hUGLU、UTP相关性无显著性意义(P>0.05)。24h内生肌酐清除率与血糖水平呈负相关,相关性具有显著性意义(P<0.05)。在年龄分组中,第1组患者UGLU水平最高,24h尿糖水平第1组与第3组、第5组之间有显著性差异(P<0.05);Ccr第2组与第3、4、5组之间有显著性差异(P<0.05);24hUTP只有第1组与第5组之间有显著性差异(P<0.05)。并且随着年龄增长,Ccr降低,UTP升高,说明年龄愈小,病史愈短,对肾脏损害愈小。结论:糖尿病肾病与血糖浓度、DM病程、是否有效控制血糖浓度密切相关。及早发现DM,有效控制血糖浓度,将对有效减少糖尿病肾病的发生,延缓DM的病程发展,提高患者生活质量,降低医疗费用具有重要的意义。
Objective:To investigate the correlation of blood glucose and kidney damage index in type I1 diabetic mellitus(DM ) patients, Methods: Patients with type Ⅱ DM at our hospital in 2010 were as the research object Their blood glucose (GLU),urine glucose(UGlu), 24h endogenous creatinine clearance rate(Ccr) and urinary protein(UTP) were measured respectively by using the OLMYPUS 5400 automatic biochemical analyzer in which the data were statistically analysed by SPSS 11.5 software. Results: With the rise of GLU, UGlu, UTP and UTP were the highest (0.93mg/L) in Glu levels between 11.0 and 12.gmmol/L group. The UGLU was the highest (11.75mmol/L) in Glu levels between 13.0 and 15.0mmol/L group. There was no significance between the Glu levels, the 24hUGLU and UTP (P〉0.05). 24h Ccr were negatively correlated with Glu levels in which there was significant(P〈0.05). Among age group, the UGLU of group 1 was the highest, and 24h UGlu between the group 1, the group 3 and group 5 was significant(P〈0.05); Ccr between the group 2, the group 3, the group 4 and the group 5 was significant(P〈0.05); 24hUTP between group 1 and group 5 was only significant(P〈0.05). Along with the age growth, Ccr decreased and UTP increased,which indicated the younger and more short process of disease, the less damage to the kidney. Conclusion: Diabetic nephropathy was closely related to the blood glucose levels, DM process and whether to effectively manage the Glu levels.There was an significance to early detecting DM and effectively controlling Glu levels,which will effectively reduce the incidence of diabetic nephropathy,slow the progression of DM, improve patient quality of life and reduce health care costs.
