玻璃酸钠联合复方倍他米松关节腔注射在膝骨关节炎中的应用

The Application of Sodium Hyaluronate Compound Betamethasone-articular Injection in Knee Osteoarthritis

目的:探讨玻璃酸钠联合复方倍他米松关节腔注射在膝骨关节炎中的应用效果。方法:选择笔者所在医院2010年6月-2011年12月住院的原发性骨性关节炎患者50例,随机分为试验组和对照组,每组25例。两组均采用膝关节髌骨上缘或下缘关节腔注射,注入玻璃酸钠注射液20mg,1次/周,5次为一个疗程,试验组仅在首次加注复方倍他米松注射液1ml,余治疗方案与对照组相同。对比治疗前后症状体征评分、疼痛程度计分及膝关节功能评分与疗效。结果:两组治疗后关节活动度、压痛、肿胀、日常活动的症状体征评分均低于治疗前,试验组治疗后症状体征各项评分均低于对照组,差异均有统计学意义(P<0.05)。经治疗后疼痛症状均有不同程度改善,试验组治疗后,VAS评分低于而LKSS高于对照组,差异有统计学意义(P<0.05)。试验组总有效率高于对照组(字2=4.152,P=0.042)。结论:玻璃酸钠联合复方倍他米松关节腔注射治疗膝骨关节炎疗效优于单用玻璃酸钠,其不良反应发生率小,安全可靠,是治疗骨关节炎的一种有效方法。

Objective：To investigate the effect of sodium hyaluronate compound betamethasone-articular injection in knee osteoarthritis.Methods：From June 2010 to December 2011,50 cases hospitalized from author＇s hospital with primary osteoarthritis were randomly divided into experimental and control groups,each group had 25 cases.Both groups were injected sodium hyaluronate injection of 20 mg once a week,5 times for a course of treatment.Only at the first the time,the experimental group were also injected with betamethasone solution of 1 ml,and the other treatment was the same with the control group.Signs and symptoms score before and after treatment,the degree of pain scoring,Knee Society function score and efficacy were compared between the two groups.Results：After treatment,range of motion,tenderness,swelling,and daily activities of the signs and symptoms scores were lower than before treatment.In the experimental group after treatment,signs and symptoms score were lower than the control group,the differences were statistically significant（P0.05）.After treatment,pain symptoms were improved,the experimental group after treatment,the VAS score was lower and LKSS was higher than that in the control group,the difference was statistically significant（P0.05）.The total efficiency of experimental group was higher than that of the control group（字2=4.152,P=0.042）.Conclusion：The sodium hyaluronate compound betamethasone articular injection in the treatment of knee osteoarthritis is better than single sodium hyaluronate,it with a small incidence of adverse reactions,safety and reliability,it is an effective method for the treatment of osteoarthritis.
