非高密度脂蛋白胆固醇与冠状动脉粥样硬化相关性分析 被引量:3

摘要 目的探讨非高密度脂蛋白胆固醇(non—HDL.C)与冠状动脉粥样硬化的相关性。方法463例患者均行冠状动脉造影,采用Gensini积分及病变支数评价冠状动脉硬化程度。所有患者均在人院后12小时内采集空腹血清,检测总胆固醇(TC)、高密度脂蛋白胆固醇(HDL.C)、低密度脂蛋白胆固醇(LDL.C)、甘油三酯(TG),并计算非高密度脂蛋白胆固醇(non.HDL.C)。按照病变支数分别分为0支病变组、1支病变组、2支病变组、3支病变组,各组组间对比分析。Gensini积分进行对数转化后与血脂进行相关性分析。数据均采用SPSSl7.0进行分析。结果(1)1、2、3支病变组血清non.LDL.C水平分别为(3.39±O.83)、(3.66±0.95)、(3.94±1.07),均高于正常组(2.66±0.71)(P〈0.01)。2支和3支病变组血清non.HDL—C水平均高于1支病变组(P〈0.01)。而2支病变组与1支病变组比较,差异无统计学意义(P〉0.05)。(2)non.HDL,C、LDL.C、TC与Gensini积分对数的相关系数分别为0.250(P〈0.01)、0.175(P〈0.01)、0.203(P〈0.01)。控制任意2个因素后进行偏相关分析,non-HDL.C与Gensini积分对数的偏相关系数为0.163(P〈0.01),LDL.C、TC的偏相关系数无统计学意义。结论血清non.HDL.C与冠状动脉硬化明显相关,且较TC、LDL.C相关性更好。 Objective To explore the correlation of none high density lipoprotein cholesterol ( non-HDL-C ) and coronary atherosclerosis. Methods Coronary arteriongraphy were carried out on all the 462 patients, the Gensini score and number of branches of lesions was used to evaluate the seriousness of coronary atherosclerosis. All the patients were taken the blood with fasting within 12 hours after admission, the total cholesterol ( TC ) , high density lipoprotein cholesterol ( HDL-C ) , low density lipoprotein cholesterol ( LDL-C ) triglyceride ( TG ) were measured and non-HDL-C was calculated. They were divided into groups of 0 branches of lesions, 1 branches of lesions, 2 branches of lesions and 3 branches of lesions in terms of the numbers of branches of lesions. Contrastive analysis was performed between groups. Gensini score was used for the analysis on its correlation to blood fat. The data were analysed with SPSS17.0. Resutls ( 1 ) the serum level of non-LDL-C in groups of 1, 2, 3 branches of lesions were ( 3.39 ± 0.83 ) , ( 3.66 ± 0.95 ) , ( 3.94 ± 1.07 ) respectively, which were all higher than that in the normal control group ( 2.66 ±0.71 ) ( P〈0.01 ) . ( 2 ) the correlation index of non-HDL-C, LDL-C, TC to Gensini score was 0.250 (P〈0.01) , 0.175 (P〈0.01) , 0.203 (P〈0.01) respectively. The partial correlation with the control of any 2 factors showed, the partial correlation index of non-HDL-C to Gensini score was 0.163 (p〈0.01 ) , the partial correlation index of LDL-C and TC had no statistical significance. Conclusions The serum level of non-HDL-C is significantly correlated to coronary atherosclerosis and is more correlated compared to TC and LDL-C.
