两孔法腹腔镜胆囊切除术的改进

Some improvements of the two-port laparoscopic cholecystectomy

目的:探讨改良两孔法腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)的手术方法及应用价值。方法:回顾分析2010年11月至2011年8月为30例患者采用常规器械行改良两孔法LC的临床资料,并与同期30例三孔法LC患者进行对比分析,观察两组患者手术时间、出血量、住院时间、切口累计长度等指标。结果:两组患者手术时间、出血量、住院时间差异无统计学意义(P>0.05);切口累计长度差异有统计学意义(P<0.01),改良两孔法LC优于三孔法LC。结论:采用常规器械行改良两孔法LC具有术后疼痛更轻、切口感染率低、微创、美观等优点,可降低操作难度,缩短手术时间,提高成功率,尤其在基层医院值得临床推广应用。

Objective： To investigate the operative method and application value of modified 2-port laparoscopic cholecystectomy（LC）. Methods：The clinical data of 30 patients who underwent modified 2-port LC with regular instruments from Nov.2010 to Aug.2011 were retrospectively analyzed,and compared with 30 patients who underwent 3-port LC.The operative time,blood loss,hospital stay and total length of incisions were observed in the 2 groups. Results：The differences of operative time,blood loss and hospital stay between 2 groups were not statistically significant（P0.05）.The difference of total length of incisions was statistically significant（P0.01）,modified 2-port LC was better than 3-port LC. Conclusions：Modified 2-prot LC with regular instruments is mini-invasive and easy with advantages of less pain,incision infection rate,operative time and higher achievement ratio.This procedure should be widely used,especially in primary hospital.
