高频振荡通气在急性呼吸窘迫综合征中的应用及原理 被引量:12

摘要 高频振荡通气(HFOV)是高频通气(HFV)的一种.1980年加拿大Bohn首次用狗作高频振荡通气研究:频率为15 Hz,潮气量为15 ml,证实其可以维持正常的气体交换.同年将高频振荡通气应用于临床,并与常规机械通气比较.高频振荡通气显示了可以减少肺内分流,提高心输出量.20世纪80年代,有人在治疗新生儿呼吸窘迫综合征的同时,对高频振荡通气的肺内气体交换机制、肺损伤和通气策略进行了多方面的研究.20世纪80年代末由美国国立卫生研究院组织的高频振荡通气和常规通气的多中心研究完成.近年来人们相继提出了肺泡复张(recruitment maneuver)策略,最佳肺容量(op-timal lung volume)的概念.使高频振荡通气的临床应用日趋完善,并在美国等多个发达国家取得了临床应用的好效果.
