摘要
[目的]探讨血清脂联素与慢性乙型肝炎病毒感染肝纤维化发生发展的关系.[方法]选择30例慢性乙型肝炎患者作为研究对象,按纤维化程度分为轻度组(A组)和重度组(B组),另选30例健康人作为对照组(C组),分别测定丙氨酸转氨酶(ALT)和天冬氨酸转氨酶(AST),血清脂联素、透明质酸(HA)和Ⅲ型前胶原(PCⅢ)等指标并进行统计分析.[结果]A组、B组BMI、ALT均显著大于C组,组间比较差异有统计学意义(P<0.05);B组ALT显著高于A组,组间比较差异有统计学意义(P<0.05).A组、B组脂联素均显著低于C组,组间比较差异有统计学意义(P<0.05);HA、PCⅢ则显著高于C组,组间比较差异有统计学意义(P<0.05);B组脂联素显著低于A组,组间比较差异有统计学意义(P<0.05);HA、PCⅢ则显著高于A组,组间比较差异有统计学意义(P<0.05).Spearman相关分析显示:血清脂联素水平与ALT、HA、PCⅢ均呈显著负相关(r值分别为-0.37、-0.52和-0.65,P<0.05).[结论]血清脂联素与慢性乙型肝炎的纤维化进展有关,具有一定的抗纤维化作用.
[Objective] To explore the relationship between adiponectin and the genesis and development of liver fibrosis in chronic hepatitis B virus infection. [Methods] Thirty patients with chronic hepatitis B were selected as research subjects. According to the degree of fibrosis, all patients were divided into mild group(group A) and severe group(group B). Thirty healthy people were selected as control group(group C). Alanine transaminase (ALT), aspartate aminotransferase(AST), serum adiponectin, hyaluronic acid(HA) and procollagen type III(PC II ) were measured and statistically analyzed. [Results] BMI and ALT in group A and group B were significantly higher than those in group C, and there were significant differences between groups( P 〈0.05). ALT in group B was significantly higher than that in group A, and there was significant difference between groups ( P 〈 0.05). Adiponectin in group A and group B were significantly lower than those in group C, and there were significant differences between groups( P 〈0.05). HA and PC III in group A and group B were significantly higher than those in group C, and there were significant differences between groups( P 〈0.05). Adiponectin in group B was significantly lower than that in group A, and there was significant difference between groups( P 〈0.05). HA and PC III in group B were significantly higher than those in group A, and there were significant differences between groups( P 〈0.05). Spearman correlation analysis showed that serum adiponectin level was significantly negatively correlated to ALT, HA and PC III (r values were -0.37, -0.52 and -0.65, P〈0.05). [Conclusion]Serum adiponectin is correlated to the progression of fibrosis in chronic hepatitis B and has a certain role of anti-fibrosis.
