摘要
[目的]探讨卵巢良恶性肿瘤组织中高迁移率族蛋白A2(HMGA2)和CD9表达水平及临床意义.[方法]40例卵巢浆液性囊腺癌、25例卵巢黏液性囊腺癌、30例卵巢良性肿瘤手术切除标本及10例正常卵巢组织常规制作石蜡包埋切片,HMGA2和CD9染色方法为EnVision免疫组化法.[结果]卵巢癌HMGA2表达阳性率明显高于,CD9表达阳性率明显低于卵巢良性肿瘤(均 P〈0.01),正常卵巢组织HMGA2表达均阴性,CD9表达均阳性;HMGA2表达阳性和(或)CD9表达阴性的良性肿瘤病例的卵巢上皮均呈中至重要不典型增生.组织学分级G1、淋巴结未转移及临床分期Ⅰ期病例HMGA2表达阳性率明显低于组织学分级G3、淋巴结转移及临床分期Ⅳ期病例(P〈0.05或P〈0.01);但CD9表达则相反(P〈0.05或P〈0.01); HMGA2表达阳性病例术后生存期明显低于阴性表达病例,但CD9表达阳性病例术后生存期明显高于阴性表达病例(均P〈0.01); HMGA2阳性表达或CD9阴性表达与术后生存率呈负相关,与死亡率呈正相关,是危险因素,均为独立的预后因子.[结论]HMGA2和CD9表达与卵巢上皮性癌发生、临床生物学行为及预后有密切关系.
[Objective] To explore the expression of high mobility group protein A2 (HMGA2) and CD9 in benign and malignant ovarian tumor and its clinical significance. [Methods] EnVision immunohistochemical method was used to determine the expressions of HMGA2 and CD9 in routinely paraffin-embedded sections of surgical resection specimens from 40 cases of ovarian serous cystadenocarcinoma, 25 cases of ovarian mucous cystadenocarcinoma, 30 cases of benign ovarian tumor and 10 normal tissues. [Results] The positive rate of HMGA2 expression in ovarian cancer was significantly higher than that in benign ovarian tumor( P 〈0.01), while the positive rate of CD9 expression in ovarian cancer was obviously lower than that in benign ovarian tumor( P 〈0.01). The expression of HMGA2 in normal ovarian tissues was negative, while the expression of CD9 was positive. Ovarian epithelium in benign tumor cases with the positive expression of HMGA2 and/or the negative expression of CD9 was middle to severe atypical hyperplasia. The positive rate of HMGA2 expression in cases with histological grade G1, non-metastasis lymph node and clinical stage I was significantly lower than that in cases with histological grade G3, lymph node metastasis and clinical stage IV ( P 〈0.05 or P 〈 0.01), hut the expression of CD9 was the opposite. The postoperative survival rate in cases with the positive expression of HMGA2 was significantly lower than that in cases with the negative expression of HMGA2, while that in cases with the positive expression of CD9 was significantly higher than that in cases with the negative expression of CD9(all P 〈0.01). The positive expression of HMGA2 or the negative expression of CD9 were negatively related to the postoperative survival rate, hut positively related to the mortality rate, so they were risk factors and independent prognostic factors. [Conclusion] The expressions of HMGA2 and CD9 are closely related to the carcinogenesis, clinical biological behaviors and prognosis of ovarian epithelial cancer.
