摘要
[目的]探讨肥胖症患者的骨密度(BMD)与体重指数(BMI)、脂肪量(FAT)、瘦体质量(LEAN)分布频律及其相关性.[方法]选择肥胖症患者150例(肥胖组),门诊健康体检者150例(对照组),比较两组BMI、FAT、LEAN等指标,并分析其相关性.[结果]肥胖组男性的上肢、下肢、躯干、全身FAT与脂肪百分比(FAT%)和躯干与全身LEAN、BMD均显著高于对照组男性,其差异均有统计学意义(P<0.01),而肥胖组男性上肢和下肢LEAN、BMD与对照组男性比较差异无统计学意义(P>0.05).肥胖组女性的上肢、下肢、躯干、全身FAT和FAT%、LEAN、BMD均显著高于女性对照组,其差异均有统计学意义(P<0.01).男性、女性全身BMD与年龄呈负相关,与BMI、FAT、LEAN、FAT% 、躯干部FAT 均呈正相关(P值均<0.01),男性各变量间的多重回归分析显示男性的年龄、BMI、LEAN是影响BMD变异的主要因素,其中LEAN对男性BMD的影响程度最大.[结论]肥胖症患者体质成分变化主要为身体各部位脂肪组织显著增多;男、女性均以躯干部位脂肪异常增加比较明显;FAT、LEAN等体质成分变化对BMD有明显的影响.
[Objective] To explore the body mineral density ( BMD), body mass index (BMI), fat mass (FAT), lean body mass(LEAN) and body constitution compositions in obesity patients and their correlation. [Methods] Totally 150 obesity patients(obesity group) and 150 healthy subjects(control group) were selected. The parameters such as BMI, FAT and LEAN were compared between two groups. The correlation was analyzed. [Results] FAT and FAT% of arms, legs, trunk and total body of male patients in obesity group were markedly higher than those in control group, and there were significant differences( P 〈0.01). LEAN and BMD of trunk and total body of male patients in obesity group were markedly higher than those in control group, and there were significant differences( P〈0.01). There was no significant difference in LEAN and BMD of arms and legs of male patients between obesity group and control group( P 〉0.05). FAT, FAT%, LEAN and BMD of arms, legs, trunk and total body of female patients in obesity group were markedly higher than those in control group, and there were significant differences( P 〈0.01). BMD of total body of mate and female patients had negative correlation with the age, but positive correlation with BMI, FAT, LEAN, FAT% and FAT of trunk(all P 〈0.01). Multiple regression analysis of each variable of male patients showed that the age, BMI and LEAN of male patients were the main influential factors of BMI, and the effect of LEAN on BMD was the most. [Conclusion] The main change of body constitution compositions of obesity patients is the significant increasing of fat tissue in all parts of body. The normal increasing of FAT in trunk of both male and female patients is more obvious. The changes of body constitution composition including FAT and LEAN have obvious effect on BMD.
