摘要 目的通过检测急性髓细胞性白血病(AML)中脑膜瘤1基因(MN1)表达,探讨其与AML预后的相关性。方法使用荧光定量聚合酶链式反应(RTQ-PCR)定量检测216例AML的MN1、同源异型盒基因5(HOXA5)、ETS相关基因(ERG)、核磷蛋白(NPM1)4种基因的表达,分析MN1表达量与AML治疗效果、预后相关因素、分型等的相关性。结果初治AML的MN1表达量比缓解AML高(P<0.05);初治急性早幼粒细胞白血病(M3)型比非急性早幼粒细胞白血病(非M3)型低(P<0.05);初治AML一次标准诱导治疗完全缓解(CR)组比未缓解(NR)组低(P<0.05)。MN1表达量在初治AML年龄≥60岁、<60岁组及男、女组中差异无统计学意义;MN1表达量与血象(WBC、Hb、PLT)、骨髓原始细胞比、免疫表型无相关性;Logistic回归分析显示,MN1表达量、年龄、血象、骨髓原始细胞比、免疫表型均与初治AML一次标准方案化疗缓解与否无相关性。MN1表达量与HOXA5、ERG及NPM1表达量无相关性。结论 MN1可能是AML的一个重要不良预后因素,并可能指导AML的个体化诊治。 Objective To investigate the relationship between the meningioma 1 ( MN1 ) gene expression and acute myeloid leukemia(AML) prognostic factors, and to study its correlation with other prognostic gene factors which affected AML expression according to the amount of expression of MN1 in AML. Methods MN1, homeobox gene 5 (HOXAS) ,ETS related gene (ERG) and nucleophosmin (NPM1) expression was measured in 216 AML adults younger by real-time reverse-transcriptase polymerase chain reaction (RTQ-PCR). Relationship between MN1 expression and the treatment, prognostic factors and type of AML were analyzed by statistical ways. Results MN1 expression level in APL was lower than that of non-APL in untreated cases, and the untreated AML expression in complete remission was lower than non-remission in one standard induction therapy. There was no significant difference between two groups of 60 years old-higher and 60 years old-lower, male and female about the MN1 expression in untreated cases. MN1 expression had no connection with hemogram (WBC, Hb, PLT) , the proportion of bone marrow progenitor cells, and immunophenotype (CD34, HLA-DR). Logistic statistical ways showed no connection with MN1 expression, age, hemogram, the proportion of bone marrow progenitor cells and immunophenotype in one standard treatment for being eased. MN1 expression had no connection with HOXAS,ERG and NPM1 expression. Conclusion MN1 may be an independent adverse prognostic factors of AML which can guide the AML individual treatment in the future.
