摘要
目的研究鼻咽癌组织中磷脂酰肌醇3-激酶(phosphoinositide 3-kinase,PI3K)、哺乳动物雷帕霉素靶蛋白(mammalian target of rapamycin,mTOR)的表达及其临床意义。方法采用免疫组化方法检测PI3K、mTOR在47例鼻咽癌和20例鼻息肉组织中的表达,回顾性研究表达与TNM分期、浸润、转移的关系。结果①鼻咽癌组及鼻息肉组中PI3K的阳性表达率分别为76.6%(36/47)、35.0%(7/20),mTOR分别为70.2%(33/47)、25.0%(5/20),差异均有统计学意义(χ2=12.36,χ2=11.6,P均<0.05);②鼻咽癌组织中,角化型鳞状细胞癌组与非角化型鳞状细胞癌组PI3K的阳性表达率分别为65.0%、85.1%,差异无统计学意义(χ2=2.61,P>0.05),mTOR分别为55.0%、81.5%,差异有统计学意义(χ2=3.85,P<0.05),I期+II期、III期+IV期组中PI3K的阳性表达率分别为80.0%、72.7%,mTOR为72.0%、68.2%,差异均无统计学意义(χ2=0.35,χ2=0.08,P均>0.05)。有无淋巴结转移组中PI3K的阳性表达率分别为70.0%、88.2%,mTOR为76.7%、58.8%,差异均无统计学意义(χ2=2.91,χ2=1.65,P均>0.05)。结论 PI3K及mTOR在鼻咽癌发生发展中起一定作用,mTOR与鼻咽癌的病理分型有相关性。
OBJECTIVE To investigate the expression and significance of PI3K(phosphoinositide 3-kinase)and mTOR(mammalian target of rapamycin) in nasopharyngeal carcinoma(NPC).METHODS Immunohistochemical Envision method was used to detect the expression of PI3K and mTOR in 47 specimen of nasopharyngeal carcinoma and 20 specimen of nasal polyp.The results were analyzed with the clinicopathological characteristics of NPC patients.RESULTS ①Positive expression rate of PI3K in NPC group and nasal polyp group was 76.6%(36/47) and 35.0%(7/20)respectively,mTOR was 70.2%(33/47)and 25.0%(5/20)respectively.There was a statistically difference between two groups(P 0.05).②In NPC tissues,positive expression rate of PI3K in groups of keratinizing squamous cell carcinoma and nonkeratinizing carcinoma was 65.0% and 85.1% respectively,and the expression difference showed no statistical significance(χ 2 =2.61,P0.05),and that of mTOR was 55.0% and 81.5% respectively,the expression difference was of statistical significance(χ 2 =3.85,P 0.05).Positive expression rates of PI3K and mTOR were 80.0% vs 72.7% and 72.0% vs 68.2% in early and advanced group respectively,and there were no statistic significance(χ 2 =0.35,χ 2 =0.08,all P0.05).Positive expression rates of PI3K and mTOR in NPC tissues with or without lymphatic metastasis were 70% vs 88.2% and 76.7% vs 58.8% respectively,and there were no statistic significance(χ 2 =2.91,χ 2 =1.65,all P0.05).CONCLUSION The results suggested that PI3K and mTOR may play an important role in the occurrence and development of NPC.mTOR has a relationship with the pathological types of NPC.
