福建畲族地区健康成人25羟维生素D及PTH水平与骨密度的关系 被引量:7

The relationship between 25-hydroxyvitamin D and PTH and BMD in healthy people in She region in Fujian

摘要 目的通过调查福建畲族地区健康人群血清25羟维生素D(25OHD)、甲状旁腺激素(PTH)水平及骨密度,分析福建畲族地区在冬季维生素D的状态并评价其与骨密度关系。方法于2010年10月至12月,调查634例畲族人群,其中男301例,女333例,年龄9-96岁,平均年龄50.14±16.07。应用罗氏公司的器械和试剂,抽血检查血清25羟维生素D、PTH。同时应用美国Osdeometer MediTech公司生产的DTX—200型骨密度仪检测前臂桡、尺骨远端三分之一BMD。根据血清25OHD水平分为<12ng/ml为严重缺乏,≥12ng/ml且<20ng/ml为缺乏,≥20ng/ml且<30ng/ml为相对不足,≥30ng/ml为充足,并应用SPSS16.0进行统计学分析。结果 634例血清25羟维生素D平均测定值15.76±8.88ng/ml,血清25OHD与性别年龄无相关,与骨密度呈正相关,与PTH无相关。维生素D严重缺乏231人占36.4%,缺乏的483人,占74.4%,相对不足的112人,占17.7%,充足的50人,占7.9%,总的维生素D不足和缺乏占92.1%。结论福建畲族健康人群在冬季的维生素D普遍不足,维生素D不足者骨密度较低,有统计学意义,与PTH没有相关。因此应重视维生素D的补充及生活方式的改变。 Objective To investigate the serum 25-hydroxyvitamin D (250HD), parathyroid hormone (PTH) , and bone mineral density (BMD) of healthy people and to analyze the 25OHD status in winter and the relationship between 25OHD and BMD in She region in Fujian. Methods Six hundred and thirty-four healthy people, including 301 males and 333 females, aged from 9 to 96 years old with the mean age of 50.14 + 16.07 years old, were investigated. Serum 25OHD and PTH were measured using the instruments and regents from Roche. BMD of 1/3 of the forearm was measured using a densitometry (DTX-200, Osdeometer Medi Tech). The levels of serum 25OHD were divided into severe deficiency as 25OHD less than 12 ng/ml, deficiency as 250HD more than 12 ng/ml and less than 20 ng/ml, relatively insufficiency as 25OHD more than 20 ng/ml and less than 30 ng/ml, and sufficiency as 25OHD more than 30 ng/ml. Results The mean value of serum 25OHD was 15.76±8.88 ng/ml. Serum 25OHD had no relationship with gender or age. It positively related to BMD and had no relation with PTH. The number of people with severe deficiency, deficiency, relatively insufficiency, and sufficiency was 231 (36.4%) , 483 (74.4%) , 112 ( 17.7% ) , and 50 (7.9%) , respectively. The total 25OHD deficiency and insufficiency accounted for 92.1%. Conclusion The insufficiency of 25OHD is common in She region in Fujian. BMD in people with 25OHD insufficiency is relatively lower than that in normal people, and the difference is statistically significant. The 25OHD insufficiency has no relation with PTH. The supplement of vitamin D and the change of life style should be focused.
