放射介入手术室医院感染的原因与对策

Causes and countermeasures for nosocomial infections in radiological interventional operating room

目的分析医院放射介入手术室医院感染现状和感染因素,针对性提出相应预防对策。方法回顾性分析医院2006年2月-2010年2月放射介入手术医院感染病例168例;对采取预防控制干预措施前(2006年1月-2008年1月)、后(2008年2月-2010年2月)医院感染情况进行比较分析;计量资料组间比较采用t检验,计数资料组间比较采用卡方检验。结果干预前放射介入手术室发生106例医院感染,感染率为19.4%,其原因主要与工作人员对医院感染认知不足有关,占44.4%,其次为医院感染监测及管理制度缺陷,占33.0%、22.6%与手术室布局不合理有关;干预后感染率为8.5%,差异有统计学意义(P<0.05)。结论介入手术室是医院感染高发区域;介入手术室布局、医院感染监测与管理制度的执行、工作人员对医院感染的良好认识、感染控制措施的认真落实是控制放射介入手术医院感染的关键因素。

OBJECTIVE To investigate the current status of nosocomial infections in interventional operating room,and to put forward countermeasures.METHODS A total of 168 patients with nosocomial infections after interventional operation in our hospital from Feb 2006 to Feb 2010 were retrospectively analyzed.Nosocomial infections before（Jan.2006 to Jan.2008） and after（Feb.2008 to Feb.2010） adopting preventive control interventions were compared.T-test was performed for inter-group comparison of measurement data and chi-square test was performed for inter-group comparison of enumeration data.RESULTS There were 106 cases of nosocomial infections occurring in radiological interventional operating room before interventions were adopted,with infection rate of 19.4%.The main reason was that staff lacked knowledge of nosocomial infections,accounting for 44.4%,followed by shortcomings of nosocomial infection monitoring and management system,accounting for 33.0%.22.6% cases were related to irrational layout of operating room.The infection rate after interventions was 8.5%,with statistically significant difference（P0.05）.CONCLUSION Interventional operating room is a department with high risk for nosocomial infections.Layout of operating room,implementation of nosocomial infection monitoring and management system,staff＇s knowledge of nosocomial infection and execution of infection control measures are key factors in controlling nosocomial infections in radiological interventional operation room.
