外科Ⅰ类切口手术围手术期预防使用抗菌药物分析

Prophylactic usage of antibiotics during perioperative period of type Ⅰ incision operation

目的了解医院外科Ⅰ类切口手术围手术期抗菌药物的使用情况,评价其合理性。方法回顾性调查分析医院2011年3、4月份出院的121例Ⅰ类切口手术围手术期抗菌药物的使用情况。结果 121例外科Ⅰ类切口手术围手术期抗菌药物使用率为100.0%;术前30min给药116例,占95.9%;抗菌药物使用前5名的分别是:头孢曲松、头孢孟多、头孢西丁、头孢替安、头孢呋辛;单一、二联用药分别占91.7%、8.3%。结论医院外科Ⅰ类切口围手术期抗菌药物使用不合理主要表现在预防使用率过高、用药级别过高、用药时间过长、更换药物频繁等,应加强监管,促进临床医师合理用药。

OBJECTIVE To investigate the application state of antibiotics during perioperative period of type Ⅰ incision operation,and evaluate its rationality.METHODS Retrospective analysis was performed to investigate the use of perioperative antibiotics in the 121 patients who underwent type Ⅰ incision surgery and discharged in Mar and Apr 2011.RESULTS The use rate of antibiotics of 121 cases undergoing type I incision surgery was 100.0%;116 cases used antibiotics 30 minutes before surgery（95.9%）;the top five antibiotics applied were ceftriaxone sodium,cefamandole,cefoxitin,cefotiam,and cefuroxime;one-drug and two-drug accounted for 91.7% and 8.3%,respectively.CONCLUSION Unreasonable factors exist in the use of antibiotics during perioperative period of type Ⅰ incision operation,such as high percent of antibiotics usage,inappropriate antibacterial selection,the prolonged continuous drug use,frequent replacement in antibacterials.So the management should be strengthened to enhance the rational use of the antibiotics.
