椎弓根螺钉固定系统的临床应用和效果分析

Clinical application and efect analysis of vertbral pedicle nail fixing system

目的 了解、评价椎弓根螺钉固定系统治疗脊柱骨折的效果。方法 75例脊柱骨折 ,用 Roy- Camille钢板、Dick钉、RF钉、和 AF钉复位固定。经 6个月至 4年 ,平均 1.5年的随访。结果 螺钉位置优良率占 86 .4% ,差占 3.7%。手术前后 X线片比较 ,平均椎体前、后缘高度分别由术前 32 %和 6 7%矫正到 94.5 %和 97%。RF钉和 AF钉固定系统矫正度可达到 96 %和 99%。结论 椎弓根 Weinstein解剖法定位较可靠 ,椎弓根螺钉钻入用钝头椎弓根探入器 ,来回约 90°转动推进的方法能顺利完成。各种内固定植入物的选用 ,目前 RF钉和 AF三维固定系统具有一定的优点 。

Objective To study and evaluate the results of vertebral pedicl nail fixing system for spinal vertebrae fracture.Methods 75 cases of spinal vertebrae fracture were treated by RoyCamille plate,Dick nail,RF and AF nail.Followup was made for 6 monuths to 4 years(average 1.5 years).Rsults The location of nail in 86.4% of the cases was good,and in 3.7% was worse.By comparison with peroperative Xray film ,the postoperative vertebrae anterior height and posterior height were corrected to 94.5% and 97% from 32% and 67%,respectively.Conclusion Weinsteins anatomical location is reliable.Up to now,RF and AF cubic fixing systems have some assured advantages and are considered to be more correspondent to biological mechanics.
