中西医结合治疗急性痛风性关节炎56例临床疗效观察

Clinical effects of the combined traditional Chinese medicine with western medicine in the treatment of 56 cases of acute gouty arthritis

目的:观察中西医结合治疗急性痛风性关节炎的临床疗效和安全性,为进一步探讨急性痛风性关节炎的治疗方法提供临床依据。方法:将112例患者随机分成观察组和对照组。观察组给予注射用血栓通冻干粉针0.45g,加入5%葡萄糖液250ml中,静脉滴注,1次/d。同时予以四妙散加味治疗,对照组给予布洛芬胶囊0.2g/次,3次/d,饭后服;疼痛缓解后,以别嘌醇片口服,0.1g/次,3次/d。两组疗程均为2周。观察两组治疗前后临床疗效、主要症状积分改善情况以及血尿酸(BUA)、总胆固醇(TC)、甘油三酯(TG)、血沉(ESR)与白细胞(WBC)等实验室检测指标的改善情况。结果:观察组疗效优于对照组(P<0.05),差异存在显著性意义(P<0.05),关节疼痛积分与关节肿胀积分差异均存在显著性意义(P<0.05),观察组能显著改善临床症状与体征(P<0.01);治疗组治疗后,治疗组的BUA、TC、TG等实验室检测指标与对照组经治疗后之间比较均有极显著性差异(P<0.01)。结论:中西医结合治疗急性痛风性关节炎可取得理想的临床疗效,值得临床推广运用。

Objective： To evaluate the clinical effects and safety of combining traditional Chinese medicine with western medicine in the treatment of acute gouty arthritis so as to privide clinical basis for further investigation of the treatment of acute gouty arthritis.Methods： 112 patients with acute gouty arthritis were randomly divided into the observation group and the control group.Patients in the observation group were intravenously injected with 0.45 g sanqi panax notoginseng freeze-dried powder injection added into 250 ml glucose liquid with the concentration of 5%,once a day.At the same time,they were given modified powder of four wonderful herbs;while the control group were treated with orally ibuprofen capsule,0.2 g a time after meal,3 times a day.The course of treatment for both groups was 2 weeks.The experimental indicators of clinical effects,the improvement of the main symptoms scores,BUA,TC,TG,ESR and WBC of both groups before and after the treatment were observed.Results： To compare the total effective rate of both groups,the observation group was higher than the control group,which was of significant difference（P〈0.05）;there was remakable difference in joint pain scores and scores of joint swelling between two groups（P〈0.05）;clinical symptoms and physical signs were greatly improved in patients of the observation group（P〈0.01）;there was significant difference in the experimental indicators of BUA,TC and TG in both groups after the therapy（P〈0.01）.Conclusion： Satisfactory clinical effects can be achieved in the treatment of acute gouty arthritis with combined traditional Chinese medicine with western medicine.The way of treatment is worthy of clinical application.
