2型糖尿病患者家庭功能与血糖控制情况相关性研究

Relationship between Family Function and Glycemic Control in Patients with Type 2 Diabetes Mellitus

目的探讨家庭功能情况与2型糖尿病患者血糖控制情况之间的关系,为糖尿病的防治提供依据。方法采用单纯随机抽样的方法,在龙华社区卫生服务中心糖尿病患者管理库中抽取2型糖尿病患者100例,根据其血糖水平按照中国2型糖尿病防治指南(2010版)的控制目标(空腹血糖3.9～7.2 mmol/L,随机血糖≥10 mmol/L,糖化血红蛋白<7.0%)分为血糖控制良好组(52例)及血糖控制不良组(48例),分别对两组患者的一般情况、运动及饮食情况、服药情况及家庭功能情况进行问卷调查,并对所得结果进行统计学分析。结果多因素Logistic回归分析显示,家庭功能是影响血糖控制情况的重要因素(P<0.01)。家庭功能良好的患者(57例)中,饮食控制合理者19例(占33.3%);而在家庭功能中-重度障碍的患者(43例)中,饮食控制合理者仅4例(占9.3%),二者比较差异有统计学意义(P<0.05)。结论家庭功能是影响糖尿病患者血糖控制的重要因素,良好的家庭功能有助于糖尿病患者的饮食控制。在糖尿病的社区管理中,要理想地控制患者的血糖,达到糖尿病二级预防的目的,改善患者的家庭功能有着重要的意义。

Objective To explore the relationship between family function and glycemic control in type 2 diabetic patients in order to provide evidence for the prevention and treatment of diabetes.Methods 100 type 2 diabetic patients selected from Longhua community health care center by using simple random sampling method were divided into glucose well controlled group（52 cases） and glucose poorly controlled group（48 cases） in accordance with the control target（fasting blood glucose 3.9~7.2 mmol/L,random blood glucose≥10 mmol/L,glycated hemoglobin7.0%） in Type 2 Diabetes Prevention Guidelines of China（2010 version）.General data,exercise,diet,medicine treatment and family function of the two groups were statistically analyzed by using questionnaire survey.Results Multivariate Logistic regression analysis showed that family function was an importance factor affecting glycemic control（P0.01）.Among the patients with good family function（57 cases）,19 cases had reasonable diet control（33.3%）.But among the patients with poor family function（43 cases）,just 4 cases had reasonable diet control（9.3%）,and the difference between the two groups was statistically significant（P0.05）.Conclusion Family function is an important factor for the control of blood glucose in patients with type 2 diabetes.Good family function is conducive to diet control for diabetic patients.In diabetic management in community,it is of great importance to control blood glucose to achieve the target of secondary prevention and to improve family function.
