摘要
目的探讨定量脑电图(qEEG)在新生儿缺氧缺血性脑病(HIE)疗效评价中的价值。方法选取HIE患儿94例,临床分为轻、中、重度三组,在基本支持和对症治疗的原则下,实行脑部营养疗程法,共进行3个疗程,治疗前和治疗过程中的每个疗程结束后均进行脑电图(EEG)检查,在常规EEG的基础上进行qEEG的数据分析,以δ频段(0.5~3.9 Hz)的功率谱值为疗效观察指标,进行对比。结果轻度组、中度组、重度组HIE患儿治疗前、第1疗程、第2疗程及第3疗程EEG检查结果异常率组内比较,差异均有统计学意义(P<0.001)。3组HIE患儿第1疗程和第3疗程EEG检查结果异常率组间比较,差异均无统计学意义(P>0.05)。3组治疗前和第2疗程EEG检查结果异常率组间比较,差异均有统计学意义(P<0.05)。轻度、中度、重度HIE患儿Fp1、Fp2、C3、C4、T3、T4、O1、O28个部位治疗前及治疗过程中qEEG检查功率谱值比较,差异均有统计学意义(P<0.001)。结论 qEEG可成为HIE疗效评估的一项客观指标。
Objective To investigate the value of qEEG in assessing the efficacy of treatment for neonatal hypoxic-ischemic encephalopathy(HIE).Methods Totally 94 newborns with HIE were divided into mild,moderate,and severe HIE groups.All of them received basic supports and symptomatic treatment,and then received three courses of nutrition therapy for brain performance.qEEG was performed in addition to conventional EEG to observe the changes of brain performances.Results Among the three groups,the abnormal rates of EEG before treatment and during the 1st,2nd,and 3rd courses showed significant difference(P0.001);the abnormal rates of EEG in the first course and the third course were not significantly different(P0.05);and the abnormal rates of EEG before treatment and in the second course were significantly different(P0.05).As for the power spectral value,qEEG confirmed that Fp1,Fp2,C3,C4,T3,T4,O1,and O2 were significantly different before and between the treatment among these three groups(P0.001).Conclusion qEEG can provide objective information in assessing the treatment for neonatal HIE.
