药学干预对特殊使用抗菌药物临床应用的影响

Effects of Pharmaceutical Intervention on Clinical Application of Antibacterial Agents for Special Use

目的:结合特殊使用抗菌药物申请单及用药数据分析药学干预前后2个月的特殊使用类抗菌药物的应用情况及趋势,为临床合理使用特殊抗菌药物提供参考。方法:收集医院HIS系统中2011年1、2月与2011年4、5月特殊使用类抗菌药物的用药数据及特殊抗菌药物申请单,对销售金额、数量、用药频度和分布科室、申请单应用情况等进行统计、分析。结果:用药金额居前3位的是第4代头孢菌素、碳青霉烯类、第3代头孢菌素。重症监护室(ICU)、干部病区、呼吸内科应用特殊使用类抗菌药物的品种数与用药频度最高。干预后2个月的特殊使用抗菌药物使用数量、用药金额均明显降低,标本送检率提高,申请单填写更加规范。结论:通过药学干预加强了对特殊使用抗菌药物的管理,规范特殊抗菌药物的使用是提高抗菌药物合理应用、减少细菌耐药性的重要措施。

OBJECTIVE：To analyze the status quo and tendency of the use of antibiotics for special use before and after intervention（2 months） by reviewing the application form of antibiotics and the drug use data so as to provide references for clinical rational use of antibiotics for special use.METHODS： The utilization data and application forms of antibacterial agents for special use during Jan.-Feb.and April-May of 2011 were collected from hospital HIS system of our hospital.A statistical analysis was performed on the consumption sum,consumption amount,DDDs,distribution of departments,filling of application forms etc.RESULTS： The fourth generation cephalosporines,carbopenems and the third generation cephalosporin dominated the first three places on the consumption sum list.The number of categories and DDDs of antibiotics for special use were the highest in ICU,cadres＇ ward and respiratory department.After intervention on antibiotics for special use for two months,the consumption amount and consumption sum of antibiotics reduced markedly,and the percentage of specimens sent for test increased and the writing of the application form became more standard.CONCLUSION： The pharmaceutical intervention contributed to strengthened management on antibiotics.The standardization of antibiotic use is an important measure for rational antibiotic use and reduction of antibiotic resistance.
