2009—2010年急性上呼吸道感染患者用药合理性调查

Rationality of Drug Use in Patients with Acute Upper Respiratory Infection During 2009-2010

目的:了解2009—2010年空军总医院急性上呼吸道感染住院患者的用药情况,分析其合理性,为临床合理用药提供参考。方法:利用医院信息系统中医生工作站的病案检索功能,以上呼吸道感染为线索,收集2009—2010年上呼吸道感染病例,共118例。以Excel表格记录每例患者的性别、年龄、体温、白细胞计数、中性粒细胞百分比、C-反应蛋白(CRP)、使用的药物和住院时间等,利用描述性统计学方法中的构成比方法对数据进行统计分析。结果:118例患者白细胞计数均<10×109个/L,中性粒细胞百分比均<75%,但CRP>10 mg.L-1者为58例,占49.15%(58/118);住院日<7 d者88例,占74.58%。抗菌药物和抗病毒药使用率分别为34.90%(156/447)和32.89%(147/447),2种抗菌药物和1种抗病毒药联用的方案最多,占40.56%(58/143);其次为1种抗菌药物和1种抗病毒药联用,占34.27%(49/143);再次是1种抗菌药物和2种抗病毒药联用,占9.79%(14/143)。结论:我院对上呼吸道感染患者所用的治疗方案对多数患者合适,但也存在着滥用抗菌药物的现象,应引起重视。

OBJECTIVE： To probe into the drug use in patients with acute upper respiratory infection in General Hospital of Air Force during 2009-2010 and analyze its rationality so as to provide references for clinical rational drug use.METHODS： With respiratory infection as clue,118 patients with acute upper respiratory infection during 2009-2010 were collected by using the retrieving function of doctors＇ workstation of hospital information system on medical records.Excel form was used to record patients＇ gender and age,body temperature,white blood count（WBC）,percentage of neutral granulocytes,C-reactive protein（CRP）,drugs received and length of hospital stay etc.These data were analyzed statistically by using constituent ratio in descriptive statistics.RESULTS： Of the 118 cases,all had WBC10×109/L and percentage of neutral granulocytes 75%;however,CRP10 mg·L-1 was noted in 58（49.15%） and length of hospital stay 7 days was noted in 88（74.58%）.The utilization rates of antibacterial drugs and antiviral drugs were 34.90%（156/447） and 32.89%（147/447） respectively.The combination two antibacterial drugs with one antiviral drug took the lead with 40.56%（58/143）,followed by combination of 1 antibacterial drug with one antiviral drug [34.27%（49/143）] and one antibacterial drug with two antiviral drugs [9.79%（14/143）].CONCLUSION： The antibiotic use was rational in most of the patients with acute upper respiratory infection in our hospital;however,great attention should be paid to the abuse of antibiotics.
