摘要
目的调查各级医院各级医师对耳鼻咽喉头颈外科住院医师规范化培训基地建设的认知和意见,探索基地建设的教学方案和途径。方法分别以上海市部分设有及不设有耳鼻咽喉头颈外科住院医师规范化培训基地的三级医院、二级医院及社区卫生服务中心为调查对象。自行设计并发放调查问卷,内容包括师资队伍、理论与实践的教学方式、考核体系等。结果发放问卷100份,回收率100%。调查结果显示:在带教师资方面有67.1%选择"固定老师带教";各级医院、各级医师普遍认为基地规范化培训应以培养临床能力为主要目标,占78.8%,其中以临床操作能力、处理问题应变能力、门急诊能力占据前三位,分别占40.7%、24.1%、21.3%;教学形式包括建立病案库、操作培训、门急诊、教学查房和PBL教学相结合;考核体系以临床、实践、病例为主线进行考核更合理,占52.9%。结论以理论知识为基础,以培养临床实践能力为目标,提高带教意识,完善师资队伍,采用传统和新型教育模式相结合的方法培养临床实践和思维能力,在实践中不断完善基地规范化建设,以培养具有扎实专科基础的临床耳鼻咽喉头颈外科医师。
Objective To investigate cognition of Standardization of residency training bases of otolaryngology head and neck surgery in various levels of hospitals and physicians and to discuss teaching programs and approach. Methods Totally 100 surgeons were selected as the investigate objectives, who majored in various levels of hospitals including third and second grade hospitals and community health centers. The questi^nnaireinc^udeteachers^the^ry^practiceteachingpr^gramsandeva^uati^nsystem.Resu^t 67.1% ofsurgeons think "fixed teacher" is better.78.8% surgeons think the main training object is the ability of clinical practice, which the top three is operation ability, respond ability, outpatient and emergency ability, while the percentage is 40.7%~ 24.1%, 21.3% separately. Teacher programs including establish cases library, practice operation, train outpatient and emergency, wards around of doctor and problem--based learning (PBL).The more reasonable main line of access system is combined theory, practice and case analysis. Conelusion In order to train resident of otolaryngology head and neck surgery professionally, the base is theory, the main object is to train clinic skills. Advance teacher's consciousness, perfect quality of teacher troop, combine traditional and new teachin~ style are all imoortant in standard the bases of otolarvn^olozv head and neck surgery.
