矮身材学生原因分析及治疗探讨

摘要 调查8.000名中、小学生中、身高值低于同年龄的同性别组儿童在X—2S以下约占0.63%。50名矮身材学生中的49名作了骨龄检查和发锌测定,29名做了生长激素测定,进行了一般体格检查、第二性征检查,调查出生史、饮食状况、学习成绩。综合评定为家族性矮小者17人,体质性生长发育迟缓者3人,属正常生长范围的晚熟儿童28人,1名属生长发育异常儿童需进一步诊断治疗。家族性矮小者骨龄与实际年龄相差一年左右;体质性生长发育迟缓者骨龄落后超过一年以上,正常晚熟者骨龄相差不超过2年;生长激素测定属正常范围;以学习成绩估计学生智力情况,大多数学生成绩处于中等水平;19名儿童的发锌测定均值为123ppm,约有10名儿童发锌值低于100ppm以下,经用锌治疗后,除2名家族矮小者外,其余身高达X—2S以内。一名17岁家族性矮小者用激素治疗后有性发育出现。 In a study of 8,000 pupils and students at Nanning, fifty cartes were found to have a height of X—2S comparing with those of the same age and sex. The ratio was 0.63%. General physical check-up, secondary sexual characteristics, history of birth condition were taken. The Zinc values in hair and its levels in serum and serum human growth hormone were measured. Hand-wrist X-rays were taken. Seventeen out of 49 cases were familial short stature(FSS), three cases were constitutional growth delay, (CGD), 28 cases were normal late children and one case of 12 years old boy was dysplasia and needs futher diagnosis and treatment. The skeletal age(SA) of most FSS was not more than ±1 year by comparision with chronologic age; The SA of CGD was less than-2 years; the normal late children in SA from ±3to ±18 months, Intelligencl of each case was evaluated with study scores, most levels of intelligencl were middle, levels of Growth hormone were within normal range, the mean of Zinc value was 123 ppm, 10 cases were lower than 100 ppm. They were given Zinc treatment. After one year, with exception of two cases of FSS, the height of the rest reached to (?)—2S, one FSS case had evidence of sexual development after the student had been given hormore.
