某综合医院专项整治Ⅰ类切口手术预防应用抗菌药物的调查分析

Investigation and Analysis of Antimicrobial Prophylaxis in Type I Incision at Special Campaigns in A Hospital

目的调查医院Ⅰ类切口手术专项管治前后抗菌药物预防应用情况,评价用药合理性,为临床合理用药提供依据。方法回顾性分别调查专项整治前2010年7～12月200例和整治后2011年7～12月216例3种Ⅰ类切口手术(腹股沟疝气、甲状腺及乳腺)住院病人资料,对抗菌药物预防用药情况进行统计和对比分析。结果抗菌药物预防使用率由99.5%下降到82.4%,平均使用日由4.255 d下降到2.667 d,术前30 min～2 h给药率由27.6%上升到65.2%,二联用药率由4.52%下降到0.56%。结论在卫生部对抗菌药物应用专项管理下,Ⅰ类切口手术围手术期抗菌药物的预防应用得到较大的改善和控制,但还存在不足之处,仍需进一步加强管理,以提高预防用药合理性。

Objective To investigate the prophylaxis application of antibiotics in type I incision before and after special campaigns in the hospital,so as to evaluate the rationality of antibiotic application,and to provide the basis for the clinical application of drugs.Methods A retrospective survey was carried out on the 3 types of I incision of 200 hospitalized patients in July to December 2010 before special campaigns and 216 hospitalized patients in July to December 2011 after special campaigns.The situation of the prophylaxis application of antibiotics was statistical and comparative analysis.Results The prophylaxis application of antibiotics rate was dropped from 99.5% to 82.4%,the average treatment time was dropped from 4.255 to 2.667 d,the rate of the first dose of antibiotic started at 0.5~2 h was rose from 27.6% to 65.2%,the rate of 2 kinds of drug combination was dropped from 4.52% to 0.56%.Conclusion The management of antimicrobial prophylaxis in type I incision got bigger improvement and control under the ministry＇s special campaigns,but also existed deficiencies,still need to further strengthen management,in order to improve the preventive medicine rationality.
