马凡氏综合征升主动脉带瓣替换术

BENTALL'S OPERATION FOR MARFAN'S SYNDROME, REPORT OF TWO CASES

本文报告2例马凡氏综合征采用带瓣人工血管行完全性手术矫治,获得成功。1例术后发生一过性谵妄和左侧肢体轻瘫,随访6～12月均恢复良好,心功能明显改善。超声对本病诊断可靠,为准确估计病变范围,术前升主动脉造影是必要的。手术成功的体会有三点:良好的心肌保护,防止出血和精细的术后治疗。文中介绍了手术和治疗方法的细节。

From April 1988 to January 1989 complete sargical correction with a composite graft was successfully performed in two patients with typical Marfan's syndrome. One patient had transient delirium and left side paresis postoperatively. Followed up for 6～12 months both were completely recovered with obvious improvement in cardiac function.Echocardiogram is reliable for the diagnosis of the disease. Aortogram of the ascending aorta is mandatory for further evaluation of the pathologic anatomy. Key steps of successful operation ares good myocardial protection, avoidance of haemorrhage and meticulous treatment in early postoperative period. Some special technique and method were described in detail.
