森林脑炎患者血清神经元特异性烯醇化酶检测的临床意义 被引量:5

摘要 目的:探讨森林脑炎患者急性期和恢复期血清神经元特异性烯醇化酶(NSE)水平的变化及临床意义。方法:采用电化学发光免疫分析法(ECL)检测90例森林脑炎住院患者血清急性期和恢复期的NSE水平,并与同期30例诊断为单纯蜱咬伤、排除森林脑炎的对照组患者进行比较。结果:森林脑炎患者急性期和恢复期血清NSE的水平分别为(19.93±11.06)ng/ml和(14.70±18.81)ng/ml,对照组血清NSE的含量为(9.69±2.69)ng/ml。森林脑炎患者急性期血清NSE的水平明显高于其恢复期和对照组(P<0.05);轻度、中度、重度三组森林脑炎患者急性期血清NSE水平分别为(11.27±2.83)ng/ml、(20.75±9.27)ng/ml和(27.77±11.82)ng/ml,重度组急性期血清NSE的水平明显高于中度组(P<0.05),中度组明显高于轻度组(P<0.01)。结论:森林脑炎患者血清神经元特异性烯醇化酶浓度明显升高,可以协助森林脑炎患者早期临床诊断,反映森林脑炎神经元损害的严重程度,可作为森林脑炎患者脑组织损伤的客观定量指标之一,血清NSE水平的检测有助于判断森林脑炎患者病情预后。 Objective : To investigate the changes and clinical significances of levels of NSE in serum from patients with acute and recovery forest encephalitis. Methods : The serum levels of NSE in 90 cases of patients with acute and recovery forest encephalitis were detected with eleetrochemilumineseence immunoassay, and the levels were compaired with the 30 cases of ticks bite patients in hospital failed to diagnosed forest encephalitis and diagnosed the upper respiratory tract infection patients as control group. Results : The levels of NSE in acute stage and recovery stage were (19.93 ±11.06) ng/ml and (14.70 ± 18.81) ng/ml, the levels of NSE in control group were (9.69 ±2.69) ng/ml. The levels of NSE in acute stage were significantly higher than those in recovery stage and control group(P〈0.05). The levels of NSE in acute stage of severe group cases were (27.77± 11.82) ng/ml, the levels of NSE in acute stage of moderate group cases were (20.75±9.27) ng/ml, the levels of NSE in acute stage of mild group eases were (11.27 ±2.83) ng/ml. The levels of NSE in acute stage of severe group cases were significantly higher than those in moderate group eases(P〈0.05). The levels of NSE in acute stage of moderate group cases were significantly higher than those in mild group cases(P〈0.01). Conclusion : The serum levels of NSE with forest encephalitis raise significantly and their levels can help patients with early clinical diagnosis. The level of NSE indicates the damage degree of neurons, and NSE in serum may be used as an objective standard of degree of brain injury. The levels of NSE in serum can help patients of forest encephalitis with the illness prognosis.
