摘要
目的调查多药耐药铜绿假单胞菌(MDRPA)中β-内酰胺酶基因和膜孔蛋白oprD2基因的存在情况。方法收集柳州市三级医院2011年1-12月标本中分离的MDRPA共20株,采用聚合酶链反应(PCR)及序列分析的方法分析A~D 4类33种β-内酰胺酶基因和oprD2膜孔蛋白基因。结果 20株MDRPA中A类β-内酰胺酶基因检出TEM(100.0%),B类β-内酰胺酶基因检出IMP(75.0%),C类β-内酰胺酶基因检出PDC(100.0%)和DHA(30.0%),D类β-内酰胺酶基因无检出;oprD2膜孔蛋白基因均未检出,提示膜孔蛋白基因编码缺失;1号株TEM基因PCR产物经测序证实为TEM-1基因,IMP基因PCR产物经测序证实为IMP-1基因;9号株DHA基因PCR产物经测序证实为DHA-1基因。结论产TEM、PDC、IMP型基因少数并产DHA型β-内酰胺酶和膜孔蛋白基因编码缺失,是该组MDR-PAE对β-内酰胺类药物产生耐药的主要原因。
OBJECTIVE To investigate the distribution of β-lactamase genes and porin protein oprD2 gene in a group of multidrug-resistant Pseudomonas aeruginosa strains.METHODS From Jun 2011 to Dec 2011,totally 20 strains of multidrug-resistant P.aeruginosa were collected from a certain tertiary hospital,then,33 kinds of β-lactamase genes and porin protein oprD2 gene were analyzed by PCR.RESULTS Of 20 strains of multidrug-resistant P.aeruginosa investigated,TEM(100.0%) were positive in class A β-lactamase genes,IMP(75.0%) were positive in class B β-lactamase genes,PDC(100.0%) and DHA(30.0%) were positive in class C β-lactamase genes,but no class D β-lactamase gene were positive and oprD2 genes were all lost.PCR product of TEM in No.1 strain was confirmed as TEM-1 by sequencing,PCR product of IMP in No.1 strain was confirmed as IMP-1,and PCR product of DHA in No.9 Strain was confirmed as DHA-1.CONCLUSION The beta-lactamase producing genes such as TEM,PDC,IMP and part of DHA as well as the loss of oprD2 gene in this group of multidrug-resistant P.aeruginosa strains plays a key role in resistance to beta-lactams.
