胰岛素不同注射方法在2型糖尿病并发感染中的临床疗效比较 被引量:7

摘要 目的探讨胰岛素的不同注射方法对2型糖尿病并发感染的临床治疗效果影响。方法选取2008年12月-2011年11月于医院进行治疗的162例2型糖尿病合并感染患者为研究对象,随机分成A、B、C、D 4组,给予每组患者不同的胰岛素和注射方法,分别为:A组采用三短一长强化注射、B组采用胰岛素泵持续皮下注射、C组采用皮下注射预混胰岛素、D组采用甘精胰岛素针皮下注射;比较4组患者的血糖达标时间及日均胰岛素用量情况以及并发症发生率。结果经研究,4组患者均能有效控制血糖,A、B、C、D组患者血糖达标时间分别为:(11.7±2.4)、(5.6±3.4)、(11.3±2.2)、(10.7±2.4)d,以B组患者血糖达标时间最短,与其他3组比较,差异均有统计学意义(P<0.05);日均胰岛素耗量分别为:(44.7±3.1)、(36.9±2.4)、(45.9±2.9)、(44.9±2.6)U/d,日均胰岛素耗量也以B组最少,与其他3组比较,差异均有统计学意义(P<0.05);4组患者的并发症比较,B组患者并发症发生率明显低于其他3组患者。结论持续胰岛素皮下注射更适宜于2型糖尿病并发感染患者的治疗。 OBJECTIVE To investigate the clinical effect of different injection methods of insulin in treatment of type 2 diabetes combined with infections.METHODS A total of 162 type 2 diabetes mellitus patients with infections from Dec 2008 to Nov 2011 were chosen as the research objects.They were randomly divided into A,B,C,and D groups.Each group was given a different insulin and insulin injection methods:group A was given with three short and a long enhanced injection,group B was given with continuous subcutaneous insulin pump,group C was given with subcutaneous premixed insulin,and group D was given with needle subcutaneous injection of insulin glargine;the blood glucose and daily insulin dose as well as the incidence of complications of the four groups were compared.RESULTS The study showed that the patients of the four groups of patients were all able to control the blood sugar effectively,the blood glucose time for group A,B,C,and D were(11.7±2.4)d,(5.6±3.4)d,(11.3±2.2)d,and(10.7±2.4)d,respectively,the blood glucose time of group B was the shortest,as compared with other three groups,the differences were statistically significantly(all P〈0.05);the average daily consumption of insulin were(44.7±3.1)U/d,(36.9±2.4)U/d,(45.9±2.9)U/d,and(44.9±2.6) U/d,respectively,group B was the least,as compared with other three groups,the differences were statistically significant(all P〈0.05);the incidence of the complications was significantly lower in group B than in other three groups.CONCLUSION Continuous subcutaneous injection of insulin can be more suitable for the treatment of type 2 diabetes blood glucose combined with infections.
