北京首钢公司脊柱关节病患者HLA-B27阳性率调查

摘要 目的调查北京首钢公司在职及部分退休职工脊柱关节病(SpA)、强直性脊柱炎(AS)患者的HLA-B27阳性率。方法制定SpA问卷调查表,对首钢公司15357例职工以非抽样普查方式进行调查。筛出疑似者进行HLA-B27及骶髂关节X线检查。SpA的分类标准采用2009年ASAS标准。结果确诊脊柱关节病68例,其中HLA-B27阳性64例,阳性率为94.12%。确诊强直性脊柱炎38例,HLA-B27阳性34例,阳性率为89.47%。结论对于疑似患者进行HLA-B27检测,有助于脊柱关节病的早期诊断及与其他疾病相鉴别。 Objective To investigate the percentage of HLA-B27 positive spondyloarthropathies (SPA) and ankylosing spondylitis (AS) among the in-service and Retired employees of Beijing Shougang Company. Methods A SpA questionnaire survey was designed and conducted among the 15 357 workers in Beijing Shougang Company. Further detections of HLA-B27 and plain films of sacroiliac joints were performed in the screened suspects. The 2009 ASAS classification criteria were used for the classification of SpA. Results Sixty-eight cases were diagnosed as SpA, among whom 64 cases were HLA-B27 positive, positive rate was 94.12%. Thirty-eight cases were diagnosed as AS, among whom 34 cases were HLA-B27 positive, the positive rate was 89.47%. Conclusion The HLA-B27 detection for suspected SpA cases is very useful for early diagnosis and differential diagnosis of spondyloarthropathy.
