螺旋CT在强直性脊柱炎骶髂关节病变诊断中的临床应用价值

Clinical application value of spiral CT in the diagnosis of sacroiliac joint disease of ankylosing spondylitis

目的探讨螺旋CT在强直性脊柱炎骶髂关节病变诊断中的临床应用价值。方法选择强直性脊柱炎骶髂关节病变患者156例,均于入院3 d内完成X线片及螺旋CT检查。结果①螺旋CT与X线片相比,对各级骶髂关节病变的显示差异均无统计学意义(P>0.05)。②CT显示关节面侵蚀130例,关节面下骨质囊变102例,骨质增生硬化105例,关节周围软组织肿胀34例,其比例均明显高于X线片(P<0.05)。结论螺旋CT具有很高的空间分辨率和密度分辨率,在强直性脊柱炎骶髂关节病变的评价中具有较高的临床应用价值。

Objective To investigate the clinical application value of spiral CT in the diagnosis of sacroiliac joint disease in ankylosing spondylitis.Methods 156 patients with sacroiliac joint disease of ankylosing spondylitis were chosen,X-ray and CT were performed within 3 days after admission.Results ①In the detection of each grade sacroiliac joint disease,X-ray and CT have no statistically difference（P〈0.05）.②CT shows 130 cases erosion of the articular surface,102 cases cystic under the articular surface,105 cases bone sclerosis,34 cases soft tissue swelling,the rates were higher than that of X-ray（P〈0.05）.Conclusion Spiral CT has high spatial and density resolution,it has high clinical value in the evaluation of sacroiliac joint disease in ankylosing spondylitis.
