238例手足口病患儿血常规及心肌酶谱动态检测及临床意义 被引量:11

Clinical significance of dynamic changes in routine blood test and myocardial enzyme test in 238 pediatric patients with hand-foot-and-mouth disease

摘要 目的探讨手足口病患儿血常规和心肌酶谱动态检测特点及临床意义。方法回顾性分析238例手足口病患儿的血常规和心肌酶谱的动态检测结果。结果手足口病患儿出疹期白细胞(WBC)、中性粒细胞(GRAN)、血红蛋白(Hb)、肌酸激酶(CK)高于恢复期(P<0.05),淋巴细胞(LYM)、血小板(PLT)低于恢复期(P<0.01)。普通型出疹期WBC、GRAN低于重症型(P=0.000),乳酸脱氢酶(LDH)、α-羟丁酸转移酶(α-HBDH)普通型低于重症型(P<0.05,P<0.01)。恢复期普通型WBC、GRAN低于重症型(P<0.05);心肌酶谱仅LDH普通型低于重症型(P<0.01)。经Spearman相关分析,LDH及α-HBDH与病情呈正相关(相关系数r分别为0.134和0.169,P值分别为0.039和0.009)。结论动态检测手足口病患儿血常规及心肌酶谱变化,对监测病情及判断预后有一定的临床意义。 Objective To investigate clinical significance of dynamic changes in routine blood test and myocardial enzyme test in pediatric patients with hand-foot-and-mouth disease(HFMD).Methods Routine blood test and myocardial enzyme test were dynamically observed and retrospectively analyzed in 238 cases of pediatric patients with HFMD.Results In acute stage of HFMD,levels of WBC,neutrophil granulocyte(NG),hemoglobulin(Hb) and creatine kinase(CK) were significantly higher than those in recovery stage(P 〈0.05),and the levels of lymphocyte and platelet were significantly lower(P 〈0.01).Comparing ordinary and severe cases,severe cases had higher levels of WBC,NG,LDH and α-HBDH with significantly statistical difference(P 〈0.05) both in acute stage.Spearman correlation analysis showed that levels of LDH and α-HBDH were closely related to severity of HFMD(r = 0.134,P = 0.039;r = 0.169,P = 0.009).Conclusion Dynamic detections of routine blood test and myocardial enzymes are helpful for understanding condition and judging prognosis of HFMD.
