经皮克氏针交叉内固定治疗掌指骨骨折

Treatment of Metacarpal and Phalangeal Fractures with Intramedullary Fixation of Kirscher′s Pin

目的:评价经皮克氏针交叉内固定技术在掌、指骨骨折中的治疗效果。方法:对94例115处手部掌骨、近节指骨新鲜骨折,在X光机监视下,采用手法整复经皮克氏针交叉内固定术治疗。从患者手的肌力、关节活动范围和影像学检查方面进行评价。结果:术后全部病例均经随访,X线片显示所有骨折均达到解剖复位或近解剖复位,平均6周拔除克氏针。按照中华医学会手外科学会上肢部分功能评定试用标准TAM评分,优16例,良70例,中8例,差0例。结论:采用闭合复位经皮克氏针内固定术,具有价格低廉、损伤小、固定确切、内固定取出方便、患者依从性高等优点。

Objective：To evaluate on the effect of treatment of metacarpal and phalangeal fractures with intramedullary fixation of Kirscher′s pin.Methods：94 cases,115 fresh fractures of metacarpal or proximal phalanx of hand,supervised by the portable x-ray radiograph,via the Manual Reduction intramedullary fixation of Kirscher′s pin.Evaluated from the aspect of muscle strength,range of motion,and the image examination of the patients′ hand.Results：All cases were followed-up,the X-ray showed anatomic or proximal reduction occurred in all the fractures.After an average of 6 weeks,the K-wire was removed.The clinical results were assessed by the upper extremity functional evaluation criteria issued by the Hand Surgery Society of the Chinese Medical Association,the clinical outcome was excellent in 16 cases,good in 70 cases,poor in8 cases and bad in 0 case.Conclusions：With less damage,reliable fixation and low price,the technique of inrtamedullary fixation of Kirscher′s pin is worth to be popularized.
