十二指肠乳头旁憩室致梗阻性黄疸15例临床分析

Clinical Analysis of 15 Cases of Obstructive Jaundice Caused by Juxtapapillary Duodenal Diverticula.

目的：探讨十二指肠乳头旁憩室致梗阻性黄疸的病因、诊断和治疗。方法：回顾总结我院１９８０ 年～１９９７年收治的十二指肠乳头旁憩室患者１５ 例。结果：全部治愈，其中１例术后出现胆瘘，经保守治疗痊愈。结论：憩室切除术易误伤胰胆管，而胃部分切除、胃空肠Ｒｏｕｘ－Ｙ吻合术联合胆道口括约肌成形术则安全有效。

Objective:To explore the etiology, diagnosis and treatment of juxtapapillary duodenal diverticula causing obstructive jaundice. Method: A retrospective review was performed of 15 cases of juxtapapillary duodenal diverticula in our hospital from 1980 to 1997. Results: All patients were cured. Biliary fistula occured in one patient and cured by conservative management. Conclusion:Diverticulectomy is difficult and may accidentally injure biliary tract and pancreatic duct, whilepartial gastrectomy and Y type gastrojejunostomy combining with papillo sphincteroplasty is safe and efficient.
