胶原膜联合同种骨块与富含血小板血浆再生牙槽嵴宽度的实验研究

摘要 目的比较犬以深冻辐照同种松质骨块与富含血小板血浆复合物(platelet-rich plasma,PRP)为支架的胶原膜引导骨再生的方法,通过动物试验证实该法的增宽牙槽嵴的效果。方法选健康成年狗15只,拔除双侧下颌第一、二双尖牙,造成较均匀的2mm左右厚度的牙槽骨嵴,继续饲养3个月待用;随机选择实验和对照侧,实验侧植入深冻辐照同种松质骨块,骨块表面覆盖PRP,对照侧植入相同形状和尺寸的深冻辐照同种松质骨块;术后2、4、6个月每次处死5只动物,完整取下实验区骨段,分别测量牙槽嵴宽度、X线片检测计算出新骨向移植骨内生长的最大深度、组织学检查统计新骨所占面积的百分比。结果实验组与对照组比较①牙槽嵴增宽量分别(1.51±0.13)mm、(2.51±0.11)mm、(3.03±0.14)mm,(1.01±0.10)mm、(1.23±0.13)mm、(1.28±0.11)mm,P﹤0.05有统计学意义;②新骨向移植骨内生长的最大深度分别是1.83mm、2.94mm、3.36mm,1.12mm、1.34mm、1.45mm,P﹤0.05有统计学意义;③新骨所占面积的百分比(36.0±1.3)%、(58.5±5.6)%、(84.3±7.8)%,(10.1±1.1)%、(25.7±4.8)%、(36.3±5.1)%,P﹤0.05有统计学意义。结论深冻辐照同种松质骨块与PRP复合物为支架的胶原膜具有引导骨再生的作用。 Objective: To compare deep freeze irradiation of the same kind of spongy bone with platelet rich plasma complexes ( platelet - rich plasma, PRP ) as scaffold of collagen membrane in regeneration of the width of alveolar ridge, through the ways of animal experiments to confirm the method for the increase of the width of the alveolar ridge effect. Methods: The maximum depth of new bone growth within the graft was calculated and the percentage of new bone area was occupied. The first and second premolars were extracted from the mandibular bone, and alveolar bone crest (2mm) was created in 15 healthy adult dogs. The dogs were randomly divided into the experimental side and the control side after 3 months. The experimental side was implanted deep freeze irradiation of the same kind of spongy bone with PRP covered, the control side was implanted the same spongy bone; The width of alveolar ridge, X - ray and histological observation were performed after the five dogs were sacrificed, each at 2, 4, 6 months. Results:The widths of alveolar ridge were ( 1.51 ±0.13) mm, ( 2.51± 0.11) mm, ( 3.03 ± 0.14) ram, ( 1.01± 0.10) mm, ( 1.23 ± 0.13) mmand ( 1.28 ± 0.11 )mm in the experimental group and the control group( P 〈 0.05 ) ; the maximum depths of new bone growth within the graft werel. 83ram, 2.94ram and 3.36ram respectively, 1.12mm, 1.34ram and 1.45MM ( P 〈 0.05 ) ; the percentage of new bone area was ( 36 ± 1.3) %, (58.5 ± 5.6) %, (84.3± 7.8) %, ( 10.1 ± 1.1) %, (25.7±4.8) % , ( 36.3 ± 5.1 ) % ( P 〈 0.05 ). Conclusion : The collagen membrane of deep freeze irradiation of the same kind of spongy bone with PRP complexes can guide bone regeneration.
