高尿酸血症对高龄糖尿病肾病早期患者肾功能的影响 被引量:8

摘要 目的探讨高尿酸血症对老年早期糖尿病肾病患者24h尿白蛋白、肾病指数的影响及其临床意义。方法将>60岁的早期糖尿病肾病患者根据血尿酸水平分为早期糖尿病肾病合并高尿酸血症(ADU)组78例和单纯糖尿病肾病(ADN)组62例。另选取老年正常人32例作为非糖尿病肾病(AN)组。测定3组空腹血糖、血肌酐、尿素氮、24h尿白蛋白(UAER)、肾病指数水平,并进行统计学分析。结果各组空腹血糖及肌酐水平比较差异无统计学意义(P>0.05)。ADU组和ADN组尿酸、肾病指数及UAER均高于AN组,且ADU组高于ADN组,差异均有统计学意义(P<0.05)。老年早期糖尿病肾病患者血尿酸水平与24hUAER和肾病指数呈正相关(r=0.401,P<0.05;r=0.387,P<0.05)。结论高尿酸血症可加重老年早期糖尿病肾病的肾小球及肾小管损伤。 Objective To discuss the effect of hyperuricemia on 24h urinary albumin,nephropathy index and clinical significance of elderly patients with early diabetic nephropathy.Methods Over 60-year-old patients with early diabetic nephropathy,according serum uric acid level divided into early diabetic nephropathy associated with hyperuricemia(ADU) group(78 cases) and simple diabetic nephropathy(ADN) group(62 cases).Also selected 32 cases elderly normal peoples were as non-diabetic nephropathy(AN) group.Determined the fasting blood glucose,serum creatinine,blood urea nitrogen,24h urine albumin(UAER),nephropathy index of three groups.Results The level of fasting blood glucose and creatinine of each group were statistically significant(P〉0.05).Uric acid,nephropathy index and UAER of ADU group and ADN group were higher than those of AN group,and those of ADU group were higher than those of ADN group,there were significant difference(P〈0.05).Elderly patients with early diabetic nephropathy in patients with serum uric acid level and 24h UAER and nephropathy index was positively correlated(r= 0.401,P〈0.05;r=0.387,P〈0.05).Conclusion Hyperuricemia in elderly patient with early diabetic nephropathy can increase the glomerular and tubular injury.
