大肠癌不同部位粘膜细胞中增殖细胞核抗原表达的研究

摘要 目的探讨增殖细胞核抗原在大肠癌及癌周不同部位粘膜细胞中的表达及意义。方法采取正常大肠10例,大肠癌远旁、近旁和癌粘膜各30例。用免疫组化染色(SP法)检测增殖细胞核抗原(proliferatingcellnuclearantigen,PCNA)的表达。结果PCNA染色阳性物质主要定位于细胞核。PCNA阳性细胞在大肠癌组织中的分布呈不同程度的异质性。正常大肠和大肠癌粘膜细胞中PCNA阳性细胞数分别为2.42±1.02和20.21±6.58,两组间比较有极显著性差异(P<0.01)。大肠癌远旁、近旁和癌粘膜细胞中PCNA阳性细胞数分别为4.26±2.14、7.43±3.26和20.21±6.58,各组间比较均有极显著性差异(P<0.01)。PCNA的表达与大肠癌的分化程度、分期和淋巴结转移有关;与组织学类型无关。结论PCNA是反映细胞增殖活性的重要指标。结合病理形态学观察,同时检测PCNA的表达,能更客观更准确地判断大肠癌的恶性程度和生物学特性。 ObjectiveTo study the significance of proliferating cell nuclear antigen (PCNA)expression in different locations of mucosa of colon cancer. MethodsS P (Streptavidin / Peroxidasa) immunohistochemical staining was used to detect PCNA in 30 cases of colon carcinoma and 10 cases of normal colon mucosa. ResultsThe positive PCNA staining was found in cell nuclei.The distribution of PCNA in colon cancer.was heterogenous in various degrees. The number of PCNA positive cells in normal colon mucosa and carcinoma was ( 2.42 ± 1.02 ) and ( 20.21 ± 6.58 ) respectively. The difference was statistically significant (P< 0.01 ). The number of PCNA positive cells in distant mucosa of the cancer,adjacent mucosa of cancer and cancer mucosa was respectively ( 4.26 ± 2.14 ), ( 7.43 ± 3.26 ) and ( 20.21 ± 6.58 ),with statistical significance (P< 0.01 ). In colon cancer the expression of PCNA was correlated with the degree of differentiation, stage and lymphatic metastases and was not related to the histological type of colon cancer. Conclusion PCNA was an important marker to reflect cell proliferation. Combined with pathological observation, the expression of PCNA can more objectively and more accurately distinguish the degree of proliferation and malignancy in colon carcinoma, as well as malignant potentiality in the cutting margin cells of colon cancer.
