摘要 目的综合评价高危型人乳头瘤病毒(HR—HPV)联合液基薄层细胞学(LCT)检查时农村女性宫颈癌筛查的临床意义,探讨大规模开展女性宫颈癌筛查的有效方式。方法采用实时荧光定量PCR对农村35~60岁女性进行HR-HPV检测,阳性者进行LCT检查,对低级别鳞状上皮内病变(LSIL)及以上病变行阴道镜下组织病理学活捡。结果采集49458例宫颈脱落细胞标本,HR-HPV阳性4080例(8.25%),其中2077例行LCT检查,结果以非典型鳞状上皮细胞意义不明确(ASC-US)及以上病变为阳性,共717例,阳性率34.52%;无上皮内病变及恶性病变(NILM)为阴性,共1360例,阴性率65,48%。63例HSIL与组织病理学活检的复合率为71.43%。结论HR-HPV联合LCT检查提高了女性宫颈癌筛查的敏感度,是大规模宫颈癌筛查行之有效的一种方式。 Objective To evaluate comprehensivly clinical application of screening UCC with HR-HPV combined LCT test and to explore the clinical significance of population screening UCC with HR-HPV combined LCT test in rural women. Methods HR-HPV test was used to the rural women aged from 35 to 60 by real-time fluorescent quantitative PCR(FQ- PCR) ,LCT was only used to the part of HR-HPV positive cases,and diagnosed with TBS. The histopathology biopsy was used to the LSIL and worse cases. Results The cervical exfoliated cells of 49 458 women were collected and there were HR- HPV positive 4 080 cases and positive rates were 8. 25%. The LCT was used to 2 077 HR-HPV positive cases. Considered NILM negative cases, ASC-US and worse positive cases, the negative and positive rates were 65. 480% and 34.52 % respec- tively. The coincidence rate with histopathology biopsy was 71.43% in cases diagnosed HSIL. Conclusion To improve the sensitivity of screening UCC by HR-HPV combined LCT test,it is an effective method to large scale screen UCA3 in women.
