摘要
目的:探讨视黄醇结合蛋白和胱抑素C以及同型半胱氨酸在糖尿病肾损伤患者早期检测的意义。方法将80例糖尿病患者按屎清蛋白排泄率(U-rinary albumin excretion rate,UAER)分为3组:A组为正常清蛋白尿组(UAER<20g/min或尿蛋白<30mg/24h)30例;B组,微量清蛋白尿组(UAER为20~200g/rain或尿蛋白30~300mg/24h)28例;C组,临床糖尿病肾病组(UAER>200g/rain或尿蛋白>0.5g/24h)22例。另择40例健康人作为正常对照(NC)组。同时检测血尿视黄醇结合蛋白(RBP)、尿微量清蛋白排泄率(UAER)、胱押素C(CysC)和同型半胱氨酸(Hey),进行相关的统计学分析。结果组问血RBP,UAER,尿RBP,Hcy和CysC值比较差异有统计学显著性意义(P<0.01)。NC组血RBP,UAER,尿RBP,Hey和CysC水平和A组相比差异无统计学显著性意义(P>0.05);均低于B和C组(P<0.01);C组血RBP,UAER,尿RBP,Hcy和CysC水平都高于A和B组(P<0.01);A组和B组相比差异也有统计学显著性意义(P<O.01)。结论RBP和CysC以及Hcy联合检测可以作为鉴另4糖尿病肾损伤患者病变程度的早期指标。
Objective To explore the significance of detection serum Cystatin C(Cys C) and Hyperhomocysteinemia(Hcy)and retinol binding proteinin (RBP) in diabetic nephropathy. Methods 80 diabetic patients with suspected DN were derided into three groups according to detect UAER level, and 40 cases as control group. The patients serum RBP, UAER, Cys C and Hcy were detected by the standard method. Results RI3P,UAER,Cys C and Hcy level in DN group was higher significantly than these in control group (P〈0. 01) ,and these index level have no statistical difference between control group and normal urinary albumin group(A group) (P〉0. 05). However, were higher in minimal urinary albumin group (B group) and DN group (C group) than control group in diagnosis of diabetic nephropathy (P〈0.01). Conclusion RBP, Cys C and Hcy could be a role .indicator for early impairment of renal function for diagnosis of diabetic nephropathy.
