老年人胃十二指肠溃疡急性穿孔的诊治分析

Clinical Analysis of elderly patients with acute gastroduodenal ulcer perforation

目的探讨老年人胃十二指肠溃疡急性穿孔的临床特点及治疗方法。方法对我院外科2002年12月到2009年12月收治的78例老年胃十二指肠溃疡急性穿孔患者的临床资料进行回顾性分析。结果本组患者中10例行保守治疗,68例行手术治疗。其中38例行单纯穿孔修补术、30例行胃大部分切除术,治愈71例、死亡3例、术后出现并发症33例次。结论老年人胃十二指肠溃疡急性穿孔容易误诊漏诊,手术并发症多。手术是主要治疗方法,应严格掌握保守治疗的指证,提高治愈率、降低病死率。

Objective To investigate the diagnosis and treatment of the acute perforated gastroduodenal ulcer. Methods From 2002 to 2008 78 old-patients with acute perforated gastroduodenal ulcers were analyzed retrospectively. Results 10 Patients in this group received conservative treatment, 68 cases with surgical treatment. 38 routine simple perforation repair, 30 patients with subtotal gastrcctomy, 71 cases were cured, 3 died, 33 cases of postoperative complications times. Conclusion Surgical operation is the first choice treatment of the acute perforated gastroduodenal ulcer. In the course of the conservative treatment the surgical operation indication should be grasped so as to improve the cure rate and reduce mortality rate.
