高度近视白内障术后高眼压观察

目的:观察高度近视患者白内障术后的眼压变化。方法:对单纯老年性白内障183例(192眼)A组,与同期进行的高度近视合并白内障患者58例(64眼),其中高度近视(-6D～-10D)37例(39眼)B组,超高度近视(>-10D)21例(25眼)C组均行白内障囊外摘出(ECCE)联合后房型人工晶状体植入术。结果:术后随访中,A、B、C组术前眼压均在正常范围,各组之间差异无统计学意义(P>0.05),A组术后1天、1周、2周、3个月的眼压与术前相比差异均无统计学意义(P>0.05),B组、C组术后1天,1周的眼压与术前相比差异有统计学意义(P<0.05),B组、C组术后2周、3个月的眼压与术前相比差异无统计学意义(P>0.05)。A组眼压升高7例(3.64%),B组眼压升高8例(20.51%),C组眼压升高6例(24%)。结论:高度近视由于眼部解剖等原因,白内障摘除术后易导致高眼压。术后注意观察眼压变化,及时发现处理高眼压,促进术后视功能恢复。

Objective：To observe the change of intraocular pressure（lOP）after extra capsular cataract extraction （ECCE）with IOL implantation surgery for cataract patients with high myopia. Methods： One hundred and eighty -three cases （192 eyes ） （group Ⅰ ） of senile cataract, and 58 cases （64 eyes） with cataract and high myopia in which cases high myopia eye （ -6D - - 10D） thirty seven cases （39eyes） （ group Ⅱ）,Super high myopia eye（ 〉 - 10D） twenty one cases （ 25eyes ） （ group 111 ） had undergone extra capsular cataract extraction （ECCE） with IOL implantation. Vision ineluding IOP between the three groups. Resuits：Preoperative IOP was normal in three groups and there was no significant difference between them （ P 〉 0. 05 ）. There was no significant difference between preoperative IOP and postoperative IOP at 1 day, lweek ,2 weeks and 3 months after operation in group Ⅰ （P 〉0.05）. In group Ⅱ and group Ⅲ, IOP 1 day and a week after operation was significantly higher than preoperation （P 〈0. 05 ）. Compared IOP 2 weeks and 3 months after operation in group Ⅱ and group m with those of preoperation , there was no significant difference （ P 〉 0. 05 ）. IOP of 7 cases in group I was raised, accounted for 3.64%, IOP of 8 cases in group Ⅱ was raised, accounted for 20. 51%, IOP of 6 cases in group Ⅲ was raised, accounted for 24%. Conclusion：Cataract patients with high myopia was easily suffered from high intraocular pressure （IOP） after extra capsular cataract extraction （ECCE） with IOL implantation surgery because of ocular anatomy etc. It necessary to observe the change of IOP and to control it in time for good visual acuity.
