椎管成型术结合Arch钢板螺钉系统治疗颈椎管狭窄的初步探讨

A Primary Study on Cervical Posterior Open-door Laminoplasty with Arch Miniplate for Cervical Spondylosis

目的观察Arch钢板系统与颈椎椎管成形术相结合治疗颈椎管狭窄的疗效。方法对1997年3月至2011年10月诊治的21例颈椎病患者,采用颈椎后路单开门椎管扩大成形基础上加用颈椎后路Arch钢板固定治疗的临床资料和影像资料进行回顾分析。结果 21例患者随访3～15个月,术前JOA评分7.9,术后评分13.5。结论利用后路固定Arch钢板系统固定颈椎管扩大成形后椎管,能有效地防止再关门和减少轴性症状,从而减少再次手术的机率。

Objective To investigate the curative effect of laminoplasty combined with Arch miniplate fixation system in treating cervical spinal canal stenosis. Methods 21 patients with cervical spondylosis were treated with posterior lateral Arch miniplate fixation of cervical spine based on unilateral open door expansive laminoplasty from March 1997 to October 2011. The clinical data and image data of 21 cases were analyzed retrospectively. Results The Japanese orthopaedic Association（JOA） scores showed significant difference between preoperative（7.9） and postoperative（13.5） evaluation in 3 to 15 months of follow-up. Conclusions Cervical laminiplasty and fixation with Arch miniplate is effective in preventing reclose of opened door and relieving axial symptom.
