TCRCα-560C/T基因多态性与原发抗中性粒细胞胞浆抗体相关性小血管炎的关系

摘要 目的探讨T细胞受体保守域α链基因(TCRCα)-560 C/T多态性与广西汉族人群中原发抗中性粒细胞胞浆抗体(ANCA)相关性小血管炎(AAV)的相关关系。方法 PCR-RFLP鉴定81例AAV患者的基因型,与190例年龄、性别相匹配的汉族健康成人作对照,行病例-对照研究、临床及病理分析。结果 (1)81例AAV患者TCRCα-560C/T存在3种基因型,CC∶TT∶CT=20∶25∶36,与健康或人(CC∶TT∶CT=45∶100∶45)比较,基因型及等位基因频率差异无统计学意义(P>0.05);(2)临床分析显示:3种基因型的患者,24 h尿蛋白定量和空腹血糖及发热发生率差异有统计学意义(P<0.05),TT基因型患者的24 h尿蛋白定量和空腹血糖均显著升高;CC基因型患者的发热发生率明显升高。(3)病理分析显示:3种基因型的患者肾脏病理活动性评分和慢性评分差异无统计学意义(P>0.05)。结论广西汉族人群中,TCRCα-560C/T基因多态性可能与AAV患者发热、24 h尿蛋白定量及空腹血糖升高相关,可能与AAV的遗传易感性不相关。
