孕11～13^(+6)周超声检查胎儿颈项透明层厚度的临床应用研究

The Study of Clinical Application about Examing Nuchal Translucency Thickness by Ultrasound at 11～13^(+6) Gestational Weeks

目的:探讨孕11～13+6周超声测量胎儿颈项透明层厚度(NT)在检出胎儿异常中的临床应用价值。方法:对1 230例孕11～13+6周的胎儿颈项透明层厚度进行测量评估,并与随访结果相对照。结果:超声共检出29例NT增厚胎儿,其中7例染色体异常,5例染色体正常,但出现颈部淋巴水囊瘤,5例胚胎死亡或自然流产,1例胎儿全身水肿,并有严重的心脏畸形,11例超声随诊至出生未见明显异常。超声测量NT厚度检出胎儿异常的灵敏度、特异度、阳性预测值、阴性预测值分别为85.7%、99%、62.1%、99.8%。结论:NT增厚在预测胎儿畸形方面,有重要的临床意义,对评价胎儿预后有很大帮助。

Objective： To evaluate the clinical application value by ultrasound examination fetal nuchal translucency thickness （NT）in detecting abnormal fetuses at 11-13＋6 gestational weeks. Methods： 1 230 cases fetal nuchal translucency were measured between 11-13＋6 gestational weeks with serial follow- up ultrasound examinations until neonate. Results：29 fetuses with increased nuchal translucency thickness had been identified by ultrasound . Among them 7 chromosome ab- normal fetuses had been identified,and 5 fetuses of normal chromosome had cystic hygroma of the neck in follow up. 5 fetu- ses embryonic death, 11 fetuses were detected by uhrasound to delivery and identified to be normal. The sensitivity, speci- ficity,PPV and NPV of estimating nuchal translucency thickness by ultrasound in detecting abnormal fetuses are85.7% , 99% ,62.1%,99.8%. Conclusion： NT thickening has higher clinic significance in screning for abnormal fetuses, and may be helpful in predicting the prognosis.
