脾结核的CT表现及诊断价值

CT diagnosis and classification of splenic tuberculosis

目的探讨脾结核的CT诊断价值及分型。方法回顾性分析20例经病理或临床证实的脾结核患者的CT及临床资料。结果本组中活动型脾结核17例,其中,粟粒状病灶5例、结节状病灶9例、冷脓肿形成3例,非活动型脾结核3例。15例患者脾脏增大,CT平扫:钙化病灶为高密度,其余病灶均为低密度,病灶常多发,边界多清晰;CT增强扫描:病灶多不强化或轻度强化,边缘廓清,冷脓肿边缘可见环状强化。20例均伴有脾外结核。结论 CT对脾结核的诊断及分型有较高的价值,可为临床治疗提供依据。

Objective To explore the diagnostic value and classification of CT about splenic tuberculosis. Methods CT findings were retrospectively reviewed in 20 patients with splenic tuberculosis proved by pathology or clinical diagnosis. Resuits Seventeen cases were active. In the active team, the lesions were military in five ones, nodosity in nine ones, cold abscess in three ones. Inactive splenic tuberculosis were three cases. Spleen enlargement was seen in fifteen patients. CT scan showed low-density except calcification. Most of the splenic tuberculosis were found as a multiple, well-defined mass. On enhanced CT, most of the lesions presented none or slightly enhancement. The edge of cold abscess may be as circle-surrounded enhancement. Tuberculosis was found in other places of body except spleen in twenty. Conclusion The diagnostic value and classification about splenic tuberculosis with CT have a high value, and it could guide clinical treatment.
