乳腺癌术后化疗患者应对方式与家庭亲密度及适应性的相关性分析

Correlation analysis of coping style with family cohesion and adaptation among postoperative patients with breast cancer undergoing chemotherapy

目的探讨乳腺癌术后化疗患者应对方式与家庭亲密度、适应性的相关关系。方法采用医学应对问卷与家庭亲密度、适应性量表，对300例乳腺癌术后化疗患者进行横断面调查。结果乳腺癌患者面对得分（18．53±1．44）低于国内常模（19．39±3．04）分；而屈服和回避得分[（14．67±1．10），（16．38±1．63）分]高于常模[（8．47±3．58），（14．45±2．97）分]，差异均有统计学意义（t值分别为4．66，29．36，10．54；P〈0．01）。患者家庭亲密度三个维度得分均高于国内常模；患者家庭理想适应性与适应性不满意程度低于国内常模，差异均有统计学意义（P〈0．01）。患者屈服和回避应对方式分别与实际亲密度、亲密度不满意程度、实际适应性和适应性不满意程度的相关性有统计学意义（r=-0．14—0．25，P〈0．05或P〈0．01）。结论乳腺癌患者的应对方式与家庭亲密度适应性有密切关系，护理人员可通过干预患者的家庭关系，促进患者采取有效的应对方式。

Objective To explore the relationship between coping style and family cohesion and adaptation among postoperative patients with breast cancer undergoing chemotherapy. Methods A cross-section survey was conducted among 300 postoperative patients with breast cancer undergoing chemotherapy, and Medical Coping Modes Scale and Family Cohesion and Adaptation Scale were used for evaluating. Results The average score of eonfronce in breast cancer patients was significantly higher than that in domestic norm [ （ 18.53 ± 1.44） vs （ 19.39 ±3.04）], and the average scores of resignation and avoidance were significantly lower than that in domestic norm[ （14.67 ±1.10） vs （8.47 ±3.58） ,（16.38 ±1.63） vs （14.45 ±2.97） ;t= 4.66, 29.36, 10.54, respeetively;P 〈 0.01]. The mean scores of ideal and unsatisfied family cohesion of patients were significantly higher than that in domestic norm, and the mean scores of ideal and unsatisfied family adaptation were significantly lower than that in domestic norm （P 〈 0.01 ）. A significant correlation was found and family cohesion, unsatisfied family cohesion, real family adaptation, and unsatisfied family adaptation were correlated with the coping style（r= -0.14 -0.25,P〈0.05）. Conclusions The coping style of postoperative patients with breast cancer undergoing chemotherapy is correlated with their family cohesion and adaptation, so it is importan（ to take action in the intervention to patients＇ family and. improve the coping style for patients.
