进修生消化内镜教学中存在的问题及对策

Problems and Solutions of Digestive Endoscopy Teaching for Advanced Students

目的总结进修生在消化内镜教学中存在的问题,并探讨解决的对策。方法以来自不同级别医院的42名进修生为研究对象,根据进修生内镜操作基础及进修目的等不同分为3组:基础操作组(n=12),镜下治疗组(n=23),特殊内镜组(n=7)。分析各组进修生的特点及存在的问题,分析各组考核情况。结果基本操作组进修生因主要来自乡镇级医院,多数不熟悉内镜的基本结构和原理,不熟悉胃肠道的正常解剖学和疾病的镜下表现,均存在操作不规范的问题;镜下治疗组由于多数来自县级以上医院,学历多为本科及研究生,多数对胃肠镜操作达到熟练或基本熟练程度,进修时间相对较长,其主要问题集中在实际操作机会较少。特殊内镜组的学历以研究生为主,全部来自省市级医院,均熟练基本胃肠镜操作,特殊内镜组的主要问题为实际操作机会较少。所有进修生返回原单位后能够独立开展所学技术。结论分组教学、因材施教的规范化消化内镜教学模式,能够满足来自不同级别医院进修生的需求,为医院消化内镜技术的广泛开展输送人才。

Objective To summarize the problems of digestive endoscopy teaching in students for advanced study and explore their strategy.Methods 42 students for advanced study of digestive endoscopy from different hospitals were divided into 3 groups,foundation operation group（n=12）,treatment under endoscopy group（n=23）,special endoscopy group（n=7）,according to the foundation of operation and objective of study.The characters and problems in students of the 3 groups and the results of examination were analyzed.Results The students in foundation groups were mainly from hospitals in small towns.Almost all of them were not from the families with elementary structure and principles of digestive endoscopy,The normal anatomy or disease manifest under endoscopy.Their operations were not normative.The students in treatment group were mainly from hospitals in counties.They were families with the elementary operations of digestive endoscopy.Most of them have the problem of scared of opportunities for practicing.The students in special endoscopy group were mainly from hospitals in big cities and had higher academic degrees.They were all families with the elementary operations of digestive endoscopy.Their most problem was scared of opportunities for practicing.All the students could evolve the new techniques they learned after they went back to their own hospitals.Conclusion The teaching model of grouping instruction and in accordance with the students＇ aptitudes for endoscopy teaching is helpful to satisfy the needs of different hospitals and promote the widespread evolvement of digestive endoscopy techniques.
