支气管哮喘患者呼吸阻力的测定及其评价

THE MEASUREMENT AND ASSESSMENT OF RESPIRATORY RESISTANCE IN PATIENTS WITH BRONCHIAL ASTHMA

本文对64例支气管哮喘患者进行呼吸阻力(Rrs)、肺量图、流量-容积曲线、动脉血气测定。结果表明,发作期患者有明显的气道阻塞,缓解期患者的Rrs明显高于正常预计值,提示缓解期患者的小气道功能尚未或不能完全恢复。支气管哮喘患者的Rrs与FEV_1%有明显相关,但在合并肺气肿的患者两者有较大差异。

In this study the respiratory resistance (Rrs), spirometry data,flow-volumecurve and arterial blood gases were determined in 64 ratients with bronchialasthma. These patients were divided into three groups: 20 patients were at stablestage (group A), 21 patients had an acute attack (group B), and 23 patientshad asthma accompanied by emphysema. The results indicate that: 1) the Rrs ingroups B and C were clearly higher than those in group A(P<0.01), further-more the Rrs in group C were higher compared with those in group B (P<0.01). This suggests that patients with exacerbation had marked airway ob-struction and in patients with emphysema the airway obstruction was more severe;2) the Rrs in patients with remission were much higher than their normalpredictive values and the abnormal detecting rate was 35%, suggesting that thesmall airway functions in patients with remission were still not or could notrecover completely; 3) the Rrs in patients with bronchial asthma was closelycorrelated with FEV_1%, however. the Rrs and FEV_1% were quite differentin patients with asthma accompanied by emphysema.
