经皮激光联合臭氧消融术与单用臭氧治疗腰椎间盘突出症的效果比较 被引量:1

摘要 目的:比较经皮激光椎间盘减压术联合臭氧消融术与单用臭氧消融术治疗腰椎间盘突出症的临床效果。方法:将100名腰椎间盘突出症患者随机分为激光联合臭氧组和臭氧组各50例,联合组采用经皮激光椎间盘减压术加臭氧消融术治疗,臭氧组单纯使用臭氧消融术治疗,术后根据改良MacNab标准判定两组疗效。结果:分别对两组患者在3个月和12个月时进行随访,3个月时联合组总有效率为98.0%,单纯臭氧消融术组总有效率为92.0%,联合组疗效显著优于臭氧组(P<0.05),12个月时联合组总有效率为98.0%,单纯臭氧消融术组总有效率为90.0%,联合组疗效也显著优于臭氧组(P<0.05),对两组患者术前术后两周进行视觉模拟评分法(VAS)评分比较,术前VAS无显著性差异,术后1d和2周时联合组VAS均明显低于臭氧组(P<0.05)。结论:经皮激光椎间盘减压术联合臭氧消融术治疗腰椎间盘突出症较单纯的臭氧消融术疗效更显著,更有利于消除腰椎间盘突出症患者的病痛。 Objective:To compare the clinical effect for percutaneous laser ablation combined with ozone alone in the treatment of lumbar disc herniation. Methods: loo patients with lumbar disc herniation from January 2009 to October 2010 in the hospital were randomLy divided into groups of ozone with 50 cases and ozone combined laser group with 50 patients. The combination group was taken with percutaneous laser disc decompression melting of ozone increases treatment, and the ozone group ozone was taken with ablation alone. To modify the efficacy after surgery according to modified MacNab criteria. Results : Take the following-up for the two groups of patients at 3 months and 12 months. The total effective rate of 3 months in the combined group was 98.0%, and the total effective rate in the pure ozone ablation group was 92.0 %. The combination group was significantly better than treatment ozone group (P〈0.05). The total effective rate of 12 months for the combined group was 98.0%, and the total effective rate in the pure ozone ablation group was 90.0%. The combination group was also significantly better than the ozone treatment group (P〈0.05). Take a comparison of visual analog scale (VAS) scores for the two group two weeks after the surgery, the preoperative VAS was without significant difference, and 1d and 2 weeks after the combination group were significantly lower than the ozone VAS group (P 〈0.05). Conclusion : Percutaneous laser disc decompression combined ozone ablation for lumbar disc herniation has a more obvious significant effect than ozone alone, and it is more conducive to the elimination of pain in patients with lumbar disc herniation.
