左侧卧位在巨大胎产妇剖宫产中的作用

Effect of continuous left-lateral position on macrosomia cesarean

目的探讨左侧卧位在巨大胎产妇剖宫产中的作用。方法选择36例择期行剖宫产手术的巨大胎产妇,随机分为实验组(17例)和对照组(19例)。麻醉后,对照组产妇采取仰卧位并将手术床向左倾斜15～30°,实验组产妇采取左侧卧位。比较两组胎儿娩出时间、新生儿体重和1min Apgar评分及产妇低血压发生情况。结果两组胎儿娩出时间、新生儿体重及1 min Apgar评分比较,均P>0.05,差异无统计学意义。麻醉后实验组产妇低血压发生率11.8%(2/17)低于对照组63.2%(12/19),两组比较,χ2=9.972,P=0.002,差异具有统计学意义。结论巨大胎产妇在硬膜外麻醉后采取左侧卧位,对于预防产妇低血压发生和保证胎儿的安全均具有重要的意义。

Objective To investigate the effect of continuous left-lateral position on hypotensive syndrome complicated with epidural anesthesia in maemsomia cesarean. Methods 36 patients undergoing elective cesarean because of maerosomia were randomly divided into experiment group （n = 17） and control group （n = 19）, The parturients of the experiment group took the left-lateral position after anesthesia and those in the control group conventional supine position with the bed declined to left at 15°-30°. Result In the experiment group the incidence of maternal supine hypotension was 11.8% （2/17）, significantly lower than that of the control group （63.2%, 12/19）（P = 0.002）. Conclusion Continuous left lateral position after epidural anesthesia could prevent maternal supine hypotensive syndrome after macrosomia cesarean.
