压缩雾化吸入肺泡表面活性物质治疗新生儿呼吸窘迫综合征 被引量:2

摘要 目的观察压缩雾化吸入肺泡表面活性物质固尔舒(Curosurf)治疗新生儿呼吸窘迫综合征(NRDS)的临床疗效。方法将63例NRDS患儿随机分为2组,吸入组32例,采用固尔舒压缩雾化吸入治疗;气管内组31例,采用固尔舒经气管导管注入治疗,2组其他治疗方法相同。观察治疗后的临床表现、脉搏氧饱和度(SpO2)及动脉血气变化。结果用药前患儿均有严重低氧血症,用药后1 h 2组患儿动脉-肺泡氧分压比值(A/APaO2)和动脉血氧分压/吸入氧浓度比值(PaO2/FiO2);均较用药前明显上升(P<0.01)。吸入组与气管内组比较,2组治疗效果无显著性差异(P>0.05)。结论压缩雾化吸入固尔舒治疗NRDS有显著疗效,能有效改善肺功能及肺氧合。 Objective To investigate the clinical effects of the Nebulized inhalation of pulmonary surfactant Cuosurf in treatment of neonatal respiratory distress syndrome.Methods The 63 cases of NRDS were randomly divided into 2 groups:32 cases were in PS inhalation group who were treated using the Nebulized inhalation of pulmonary surfactant Cuosurf,31 cases were in intra-airway group who were treated with trachea catheter implantation of pulmonary surfactant Cuosurf.All of the other supportive treatments were the same in the both groups.The clinical symptoms、SpO2and arterial blood gas were observed before and after the treatment.Results Hypoxemia was significant before the treatment in both groups.Compared with that before treatment,PaO2/fraction of inspired oxygen(FiO2)ratio,arterial oxygen/alveolar oxygen(a/APO2)ratio were significantly higher in both groups after 1 hour,Compared with the two groups,the treatment effects have markedly differences(P0.05).Conclusion Nebulized inhalation of pulmonary surfactant Curosurf might be effective in treatment of neonatal respiratory distress syndrome by improving lung function and lung aeration.
