液基细胞学技术结合针吸细胞学在淋巴结疾病诊疗中的应用分析

摘要 目的:探讨液基细胞薄层技术(liquid based cytology test,LCT)结合针吸细胞学在肿大淋巴结临床诊疗中的应用价值。方法:采用常规针吸细胞学(fineneedle aspiration cytology,FNAC)检查结合液基细胞薄层制片技术对166例淋巴结肿大患者进行细胞学诊断,采用中医治疗验证细胞学诊断的正确性。结果:166例淋巴结肿大经常规FNAC结合LCT诊断淋巴结反应增生65例,坏死性淋巴结炎39例,淋巴结结核62例,分别予以中医辨证治疗。以临床验证病理,其中慢性淋巴结炎、坏死性淋巴结炎和淋巴结结核诊断符合率,分别92.3%、94.9%和93.5%。结论:LCT细胞量丰富、细胞形态和涂片背景清晰结合传统涂片,有助于提高病理医师阅片的快速性和准确性,为临床治疗提供可靠依据。 Objective :To explore the application valuation of liquidbased cytology test combined with fineneedle aspiration cytology in the clinical diagnosis and treatment of swollen lymph nodes. Methods : 166 cases of swollen lymph nodes were given cytological diagnosis by adopting fineneedle aspiration cytology detection combined with Tap liquid based cytology test (LCT) technique, then to verify the exactness of cytological diagnosis with the treatment of TCM. Results:After 166 cases of swollen lymph node were given routine FNAC and conventional LCT diagnosis ,65 cases were lymph node reactivity hyperplasia ;39 cases were necrotizing lymphadenitis and 62 cases were lymph node tuberculosis. Three groups were given traditional Chinese medicine syndrome differentiation and treatment respective ly, so as to verify clinical pathology. Among them, diagnostic coincidence rates of chronic lymphadenitis, necrotizing lymphadenitis and lymph node tuberculosis diagnosis were 92.3 % ,94.9% and 93.5 % respectively. Conclusion:Abundant number, clear form and smear background of LCT cells are beneficial for pathological doctors to improve the efficiency and accuracy of reading pathology slice com bined with the traditional smear, and provide reliable basis for clinical treatment.
