高频振荡通气在新生儿呼吸窘迫综合征治疗中的应用 被引量:10

摘要 目的探讨高频振荡通气(HFOV)治疗新生儿呼吸窘迫综合征(NRDS)的疗效及安全性。方法对NRDS64例,随机分为常频机械通气(CMV)30例和高频振荡通气(HFOV)34例,比较两组患儿治疗前后的血气分析、吸人氧浓度(Fi02)、氧合指数(OI)及并发症等。结果HFOV组患儿吸入氧分数明显低于CMV组,PaCO2及氧合指数迅速下降,pH及PaO2明显上升,HFOV组发生肺气漏及慢性肺部疾病(CLD)等并发症低于CMV组(P〈O.05)。结论HFOV用于治疗新生儿呼吸窘迫综合征,是一种疗效肯定、安全性好的新型机械通气方法。 Objective To explorer efficacy and safety of the high frequency oscillatory ventilation ( HFOV ) in the treatment of neonatal respiratory distress syndrome ( NRDS ) . Methods Sixty-four neonates with RDS were randomly grouped as HFOV group ( n=34 ) and conventional mandatory ventilation group ( CMV group, n=30 ) , based on their fetal age, weight, age, and their clinical condition from Jun 2009 to Jun 2011. The blood gas analysis, the fraction of inspiratory oxygen, oxygenation and complication were detected and compared between the 2 groups. Results The fraction of inspiratory oxygen ( FiO2 ) of the HFOV group was significantly lower than the CMV group. In HFOV group, Pa ( COg ) and oxygenation index rapid declined after treatment. The pH and PaO2 increased significantly than the CMV group. The incidence of air leak and chronic lung disease ( CLD ) in the HFOV group is lower than CMV group ( P〈0.05 ) . Conclusion HFOV for the treatment of neonatal respiratory distress syndrome is an effective and good security mechanical ventilation.
